Effects of Cardiopulmonary Bypass on the Levels of Plasma Neuropeptide Y during Peri-CPB Period of Rheumatic Valvular Replacements

Abstract


Objective: Determination of rheumatic heart disease, heart valve replacement surgery cardiopulmonary bypass (Cardiopul-monary Bypass, CPB) during neuropeptide Y (Neuropeptide Y, NPY) plasma concentration observed CPB CPB period plasma nerve Peri valve replacement for rheumatic heart disease the impact of neuropeptide Y and explore its clinical significance . Methods: 20 cases of first - time elective the MORALS heart disease valve replacement surgery patients before surgery , the turnaround (CPB began after 5 min ) after the turnaround ( immediately after the cessation of CPB ) and after surgery for 24 hours , a total of four points collected blood samples . Specimens of each time point inspection routine blood test to check the platelet count ( Plt ) , hematocrit (Hct) , neuropeptide Y plasma concentrations were measured by radioimmunoassay . Results: ( 1 ) before surgery turn for the better and in turn for the better after the NPY concentration were significant rise ( P = 0.00 ) , NPY concentration than 24 hours after surgery dropped significantly after the turning point , but still higher than before surgery (P = 0.02 ) . ( 2) compared with before surgery , turnaround and turnaround , Plt quantity were significantly reduced (P = 0.00), 24 hours after surgery Plt number than transit rebounded , but still lower than before surgery ( P = 0.00) . ( 3 ) neuropeptide Y concentrations in cardiopulmonary bypass time after the turning point was a positive correlation (r = 0.54, P = 0.01). ( 4 ) the plasma NPY higher turnaround after the value of platelet decline in value is positively correlated (r = 0.48, P = 0.03). Conclusion: 1 . Cardiopulmonary bypass can lead to rheumatic heart disease and valve replacement surgery in patients with plasma neuropeptide Y were significantly elevated . Rheumatic heart disease patients undergoing valve replacement plasma neuropeptide Y concentration and platelet destruction during cardiopulmonary bypass . 3 . Shorter cardiopulmonary bypass time protection platelet , can reduce the release of neuropeptide Y , to reduce the concentration of plasma neuropeptide Y , so as to reduce the damage to the body of cardiopulmonary bypass surgery , reduce the incidence of postoperative complications .
