Abstract


Objective: To investigate serum procalcitonin (PCT) in community- acquired lower respiratory tract infections (LRTI) clinical value . Early diagnosis , clinical treatment for lower respiratory tract infections , evaluate the prognosis of patients to provide experimental basis . Methods: 67 patients selected adult community-acquired lower respiratory tract infections (LRTI) , 20 patients with non - infectious systemic inflammatory response syndrome (SIRS) , 25 cases of healthy individuals . LRTI patients with lower respiratory tract infection and the SIRS 28 cases , is not associated with SIRS group, 18 patients ; nonbacterial lower respiratory tract infection in 21 cases . ELISA quantitative determination of serum procalcitonin (PCT) concentration , comparing bacterial and non-bacterial lower respiratory tract infection in patients with PCT concentration differences ; comparing 28 cases of bacterial lower respiratory tract infection with SIRS patients and 20 patients with non - infectious SIRS differences in serum PCT patients ; dynamic detection of bacterial lower respiratory tract infections in patients after admission the first day of the third day , seventh day serum PCT concentration changes . PCT concentrations in patients with Acute Physiology and Chronic Health Evaluation (APACHE II) observed lower respiratory tract infection . Results: bacterial and non-bacterial respiratory infections compared to patients , serum PCT concentrations significant difference . According to the ROC curve selected serum PCT concentrations 0.177ug / l, forecast sensitivity 71.4% , specificity 66.7% . Bacterial lower respiratory tract infection with SIRS group compared with non- infectious SIRS group , significant difference in serum PCT concentrations , leukocyte , neutrophil percentage and C-reactive protein was no significant difference . Bacterial lower respiratory tract infection in patients admitted to hospital the first day , the third day , the seventh day of PCT concentration (ug / l) were 0.30 Guests 0.16 ; 0.19 Guests 0.11 ; 0.13 Guests 0.10 , With the remission declining , significant difference (P lt; 0.05). The seventh day of PCT concentrations compared with healthy subjects PCT concentration after admission was no significant difference ( P gt; 0.05 ) . PCT level was significantly associated with APACHE II . Conclusion : Adult respiratory bacterial infection in patients with serum PCT concentrations increased non-bacterial infection of serum PCT concentrations did not change significantly ; PCT and lower respiratory tract infections the severity of the disease have a certain correlation ; infection with SIRS serum PCT concentration ; non - infectious SIRS serum PCT concentrations did not change significantly ; the dynamic detection PCT concentration showed that with the infection control PCT concentrations returned to normal .
