Objective To study the lung local hypothermia combined protective ventilation in oleic acid in acute respiratory distress syndrome (ARDS) in animal models of lung pathological changes . Methods 18 healthy dogs were anesthetized with oleic acid - induced acute respiratory distress syndrome , the models were successfully after the animals were randomly divided into three groups , each group of six , namely: conventional mechanical ventilation group (A group , the control group ) ; low - volume low-pressure mechanical ventilation group (B ) ; regional hypothermia combined with low capacity low pressure group (C ) . 1,2,3 hours after the observation and treatment , the pulmonary surface temperature (Tl), blood temperature (Tb), extracted into the mold after 0:00 and after treatment 1, 2, 3 hours and a total of four time points femoral artery 3ml measured by radioimmunoassay. tumor necrosis factor (TNF-α), interleukin -6 (IL-6) content . After the end of the experiment , the animals were sacrificed , end-expiratory occlusion of the tracheal tube , remove the lower left lung , pathological changes in lung tissue in the light microscope . No statistically significant results ( 1 ) group C Tl , Tb 2,3 point in time after treatment was significantly lower ( P lt; 0.05 ) , A , B temperature change ; (2) B , C in lung tissue accumulated average significantly low group A , there is a significant difference ( P lt; 0.05 ) . (3) A, B, C three groups atelectasis , perivascular edema , alveolar edema , alveolar hemorrhage pathological score significant difference , A group of pathological scores were higher than the other groups , the C group had the lowest (P lt; 0.01) . (4) A serum TNF - α , IL -6 levels higher than the B, C group ( P lt; 0.05 ) , while group B than group C ( P lt; 0.05 ) . Conclusion lung local cooling combined protective ventilation can inhibit TNF -α , IL - 6 expression , alleviate oleic acid ARDS canine lung tissue injury , have certain lung protection .
