Abstract


Objective:Hepatolithiasis is a prevalent disease in China. Hepatectomy is considered to be the standard treatment for this disease. Laparoscopic hepatectomy is a procedure for the treatment of intrahepatic stones with the advantage of minimal invasiveness. However, the role of laparoscopic major hepatectomy for treatment of hepatolithiasis has not been discussed yet. This study is to evaluate the feasibility and safety of laparoscopic left semihepatectomy for hepatolithiasis by comparing it with open hepatectomy.Methods:From November,2004 to 2010,December,69 patients underwent laparoscopic left semihepatectomy for hepatolithiasis in our center. They were included in this study and called the laparoscopic hepatectomy group (LH). During the same period,107 patients with hepatiolithiasis in the left hepatic lobe received open left semihepatectomy and were called the open hepatectomy group (OH).Results:Compared with OH group, LH group has significantly decreased blood loss and less chance for blood transfusion, and similar operating time and postoperative hospital stay. The complication rate for LH group is similar with OH. as well as the residual stone rate. Conclusion:In a center with expertise of experienced laparoscopic surgeons, laparoscopic left semihepatectomy for hepatolithiasis can be more feasible and safer in selected patients than open operation, and may be the routine operation in future.
