Abstract


Objective: To investigate the the posterior cervical single door efficacy of surgery for cervical spondylotic myelopathy (cervical spondylotic myelopathy, CSM) as well as the relevant factors affecting the efficacy of surgical , to provide a reference for clinical work . Clinical data and methods : Retrospective analysis of March 2008 to November 2010 , the treatment of patients with cervical spondylotic myelopathy Affiliated Hospital of Shandong University of Traditional Chinese Medicine Spinal orthopedic surgery , all patients underwent preoperative history , physical examination and routine cervical X ray films and MRI, postoperative outpatient , phone calls, letters , and other follow-up , a total of 35 cases met the inclusion criteria . JOA 17 points scoring and Nurick cervical spondylosis spinal cord functional classification to assess and calculate the rate of improvement of the patient before surgery and the last follow - up spinal cord function . The application independent sample t test , analysis of variance statistical methods for univariate and multivariate analysis to investigate the degree of spinal cord compression , cervical cord MRI signal changes in the preoperative , preoperative function score , OPLL , developmental spinal stenosis , cervical curvature posterior single door surgical treatment of cervical spondylotic myelopathy . Results: The follow-up of 12 to 30 months , an average of 24 months , preoperative spinal cord JOA score an average of 9.73 ± 3.66 points , Nurick grading an average of 2.8 ± 1.25 Ji ; spinal JOA score last follow-up an average of 14.5 ± 2.38 points , Nurick grading average level; 1.03 ± 1.14 at last follow - up patient JOA score and Nurick grading than before surgery significantly improved, the difference was statistically significant ( P <0.01 ) . The final follow-up JOA score improvement rate of rating : excellent in 15 cases , good in 16 cases , in three cases , and poor in 1 case . Statistical analysis of the relevant factors affecting the efficacy of surgical results : preoperative spinal MRI signal changes (P <0.05), preoperative functional scores (P <0.05), postoperative improvement rate of preoperative spinal MRI cervical curvature (P <0.05); signal change , preoperative function score , cervical curvature . Conclusion: The surgical treatment of posterior cervical spondylotic myelopathy significant preoperative spinal MRI signal changes , preoperative function score , cervical curvature factors affect the surgical treatment of cervical spondylotic myelopathy , as a good indicator to determine the surgical outcomes .
