[Objective] total hip arthroplasty postoperative periprosthetic fracture risk factors , treatment , prognosis . [Methods] November 2001 - April 2009 19 periprosthetic fracture patients were studied retrospectively . Of which 7 were males and 12 females . Mean age 55 years (44 - 86 years old) . Total hip prosthesis after fracture around the Vancouver classification, the queue B1 6 cases , 12 cases of type B2 , B3 , 1 . Non-surgical treatment in 1 case, revision surgery in 13 cases, the cut complex inner fixation five cases . [Results ] The mean follow-up time of 31 months (6 - 69 months). Conservative treatment group 1 patients , 6 months after bone healing , Harris score 86 points . Revision surgery group of 13 patients , bone healing . The average healing time was 4.1 months (3 - 7 months) . Postoperative Harris score 79 points (60 - 93 ) . Cut complex internal fixation group in 5 cases, have reached the bone healing . 1 case of delayed union , the average healing time was 6.6 months ( 3-10 months) . The average postoperative Harris score 75 points (67 - 86 ) . Conclusion osteoporosis , osteolysis , local stress concentration is the main cause after total hip replacement periprosthetic fractures . Parting Vancouver type B1 , the line should cut complex internal fixation . Type B2 , B3 the line should skillet prosthesis revision surgery and allograft bone loss supplemented .
