Abstract


Objective: To investigate the hamstring tendon graft reconstruction of the anterior cruciate ligament (ACL) femur biomechanics end of the extrusion of cortical bone screw fixation, histological observation of cortical bone extrusion nail in rabbit ACL reconstruction outcome evaluation of cortical bone extrusion nail clinical effect. Methods: biomechanical experiments: fresh pig knees were eight, with four shares sheep hamstring tendon graft reconstruction of the ACL femoral and tibial tunnels extrusion with homemade cortical bone fixation nail (n = 8) is fixed; specimens were fixed in biomechanics on the test machine, and the pullout of record load - displacement curve, respectively, to test the maximum load, the 100N and 400N displacement, tensile stiffness, and failure mode data were statistically analyzed. Animal experiments: adult New Zealand white rabbits (weighing 2.5 ± 0.3Kg) 24, were randomly divided into 2 groups (n = 12), anesthesia under autologous long toe extensor tendon underwent bilateral ACL reconstruction of the knee, the tibia the end squeeze nails placed in the allograft cortical bone, respectively, in experimental animals were sacrificed at 4, 8 and 12 weeks after surgery, The bilateral knee specimens observed cortical bone extrusion nail changes in imaging and histological outcome . Clinical application: using Endobutton bone extrusion screw fixation of femoral end, the door-shaped the nail or Rigidfix fixed tibial bone ends rebuild ACL7 cases in 6 males and 1 female, with an average age of 30 years old (19 in January 2010 - April 2010 - 49 years), preoperative, three days after the operation, after a month Rolimeter measurement of anterior and X-Ray inspection. Results: The average maximum load of the cortical experimental group, 100N displacement, 400N displacement and tensile stiffness were 418 ± 50N, 6 ± 2mm, 15 ± 6mm, 41 ± 10N/mm. Statistical analysis showed that the above test data with absorbable interference screw between the difference was not statistically significant (P gt; 0.05). Extrusion of cortical bone revascularization nail in the rabbit tibial bone - tendon - bone healed well, obviously, the organization gradually creeping substitution. Postoperative All 7 patients had no infection, complications such as neurovascular injury, postoperative X-Ray bone extrusion nail gradually creeping substitution, Rolimeter measured value q test, patients with preoperative bilateral knee ipsilateral knee joint preoperative and postoperative three days, ipsilateral knee surgery before and 1 month after the tibia forward degree of difference was statistically significant (P ≤ 0.01); ipsilateral knee after surgery three days after a month, respectively, with the contralateral knee comparison of anterior degree of difference was not statistically significance (P gt; 0.05); the ipsilateral knee three days after over a month, the tibia forward the degree of the difference was not statistically significance (P gt; 0.05). Conclusion: cortical bone extrusion nail pullout load, 100N displacement, 400N displacement and tensile stiffness absorbable interference screw similar to to meet initial fixed ACL reconstruction requirements; histocompatibility good biological fixed material, pending further study.
