Abstract


Purpose in recent years , with the increase in the incidence of endometriosis , as well as raising the level of pathological diagnosis and a corresponding increase in the incidence of malignant transformation of endometriosis , caused widespread concern . Through clinical analysis of 49 cases of ovarian uterine endometriosis malignant patients to investigate the clinical characteristics and treatment of the living conditions . Methods of China Medical University Shengjing Hospital were treated between January 2002 - December 2009 , 49 cases of ovarian endometriosis malignant transformation of the patient's medical record information , follow-up . Retrospective analysis of the treatment of 49 patients , and analysis using the Kaplan-Meier method single factor in the survival rate difference in survival of the Log -rank law test , P lt ; 0.05 was statistically significant . (1) Clinical data : 49 cases of endometriosis malignant patients average age 51 years old, mainly abdominal mass ( 51.0% ) , abdominal distension, abdominal pain (44.9 %) complained of admission , a small number of patients (4.1% ) menstrual changes chief complaint of admission . (2) Statistical analysis : ① preoperative CA125 levels : preoperative CA125 divided into to ≤ 200IU/ml and gt ; 200IU/ml two groups, two groups of survival statistically significant difference ( P = 0.047 ) ; ② postoperative residual disease size, postoperative residual disease is divided into ≤ 2cm > 2cm two groups , there is a statistically significant difference ( P = 0.001 ) ; ③ The Ⅰ / II period, III / IV patients survival rate significant difference ( P = 0.027 ) ; ④ endometrial cancer and the survival rate of patients with clear cell carcinoma a significant difference ( P = 0.040) , a significant difference (P = 0.022 ; ⑤ chemotherapy patients greater than six courses , with the survival rate of patients with less than six courses ). Conclusion 1 , ovarian endometriosis certain rate of malignant endometrioid carcinoma and clear cell carcinoma , malignant pathological type . Preoperative CA125 levels , postoperative residual lesion size , clinical stage , pathological type and chemotherapy treatment and prognosis is closely related .
