Abstract


Objective To observe the pancreatic focal lesions ultrasound contrast perfusion characteristics , analysis of imaging parameters , evaluation of contrast-enhanced ultrasound diagnostic value of focal pancreatic lesions and enhanced CT contrast analysis . Method proposed in our hospital from July 2008 to May 2009 surgery the pancreas focal placeholder patients underwent ultrasound imaging , the instrument uses the GE Company LOGIQ - 9 ultrasonic diagnostic apparatus , the probe frequency 2 - 4MHz , mechanical index 0.07 - 0.10 . Patients examined fasting more than 8 hours before the test drink 500 - 800ml of water . The size of the first two-dimensional ultrasound focal pancreatic mass , echo characteristics , the edge is structured, to start color Doppler system lesions observed internal blood flow , the initial diagnosis made ??before angiography . Good lesions observed section , the contrast , Sonovue ultrasound contrast agent configuration into a suspension , the median cubital vein each bolus injection of 2.4 ml , at the same time to start the the ultrasonic instrument built-in timer , real-time continuous observed lesions enhanced changes . Contrast the whole process less than 5 minutes. Analysis : the lesions enhanced features , divided into high enhancements , enhancements, and low - enhanced and enhanced . Start the the TIC software , lesions and normal pancreatic tissue were measured initial increase , time to peak , calculated to enhance time ( peak time - the beginning of growth ) , while recording the peak intensity . Results Of the 21 cases , 10 cases of benign and malignant 11 cases . Pancreatic cancer mainly contrast the performance of low - enhanced arterial phase uneven , The benign lesions mainly for the arterial phase uniform enhanced or enhanced , shorter than normal pancreatic tissue contrast enhancement of malignant lesions , benign lesion contrast enhancement with normal pancreatic tissue no significant difference. Low arterial phase enhanced as the standard of the diagnosis of pancreatic cancer , the sensitivity was 72.7 % , specificity 80.0 % , positive predictive value of 80.0% , negative predictive value of 72.7% , and accuracy of 76.2% . The focal pancreatic lesions benign and malignant diagnostic accuracy of ultrasound contrast -enhanced CT were 81.0% , 85.7% . Conclusion Ultrasound imaging can objectively reflect the microcirculation of the lesions , to provide more information than conventional ultrasound , different pathological types of focal pancreatic lesions contrast performance , differential diagnostic value of contrast-enhanced ultrasound . Benign and malignant diagnosis rate similar to contrast-enhanced CT has a good prospect for clinical application .
