Abstract


Objective: To establish a rabbit model of hindlimb acute ischemic rhabdomyolysis; explore the extent and scope of the value of MRI early diagnostic evaluation rabbit rhabdomyolysis disorder; provide experimental reference for the clinical application of MRI to determine the indications for amputation treatment of rhabdomyolysis. Methods: 34 New Zealand white rabbits, all of a rabbit hindlimb model of acute ischemic rhabdomyolysis. Before surgery, 1, 2, 3, 4, 5, 6, 7 hours magnetic resonance T1WI, T2WI, fat suppression, T2map, DTI scanning and post-processing to do imaging, get the vastus lateralis muscle T2 values ??and ADC value; preoperative and scanned the vastus lateralis muscle biopsy and creatine kinase, lactate dehydrogenase, creatinine and lactate detection. After statistical analysis, comparison of relevant indicators to determine the the the ischemic side vastus lateralis contralateral vastus lateralis and ischemic side vastus lateralis differences change over time. Result: the the rabbit hindlimb acute rhabdomyolysis model technical success rate of 100%. Seven hours after creatine kinase (CK), lactate dehydrogenase (LDH) values ??were significantly higher than preoperative (P lt; 0.01), creatinine, lactate values ??were higher than those before (P lt; 0.05). Pathological results: the ischemic side vastus lateralis preoperative and 1-5 hours after surgery no significant difference, the granular degeneration after six hours, seven hours after rhabdomyolysis fuzzy. MRI results: two hours after the ischemic side vastus lateralis T2WI fat suppression sequences began to appear progressive signal increased; seven hours after the ischemic side vastus lateralis muscle contralateral was slightly higher signal: T1WI, T2WI fat suppression water samples showed high signal sequences; the ischemic side vastus lateralis T2 values ??contralateral T2 value ratio increased linearly within 1-6 hours, six hours from the decline (P lt; 0.05); ischemic side vastus lateralis muscle ADC values ??and the contralateral ADC value ratio increases linearly 1-5 hours, 5 hours decreased (P lt; 0.05). Conclusion: ① pathology results confirmed the of rabbit vastus lateralis muscle completely ischemic seven hours rhabdomyolysis. 2 rhabdomyolysis MRI signal changes with edema; to rhabdomyolysis stage from the onset of ischemia, edema accompanied which two hours; fully ischemia may manifest as increased signal on FSE-T2WI fat suppression sequences. ③ T2 values ??can be quantified to reflect striated water molecules completely ischemic decline from 6 hours to change the content of the water molecules rhabdomyolysis; ADC value can be quantified to reflect the striated water molecules; decline from the five hours of completely ischemic Description the degree of diffusion of water molecules change imminent rhabdomyolysis. ④ T2 values ??and ADC values ??quantified to reflect edema change may indirectly reflect rhabdomyolysis; This is the extent and scope of the clinical application of MRI early diagnostic evaluation rhabdomyolysis, determine the the amputation treatment of indications provide a new choice.
