Abstract


Background and Objective: Breast cancer is a common malignancy of women and the cause of death of one of the world's annual increase of about 1.2 million breast cancer patients, about 25 million people a year die from breast cancer. With westernized lifestyle in recent years, the incidence of breast cancer in our country a clear upward trend in breast cancer in the city has become the most common tumor in women, after lung cancer, breast cancer mortality in female cancer mortality in the first place, while the transfer is the main reason for death of breast cancer patients. Currently considered one of the main reasons for postoperative metastasis is not detected in the preoperative micrometastases or circulating tumor cells (Circulating tumor cells, CTC) into the blood, so look for a sign of early tumor specific marker for metastasis early detection of micrometastases in the subclinical stage cancer has important clinical value, is very important to further explore the mechanisms of breast cancer invasion and metastasis, and to find new treatments to improve the effect of breast cancer treatment, and to improve the survival rates of breast cancer . Transfer adhesion genes (Metadherin, MTDH) is an important gene involved in tumor invasion and metastasis, the encoding gene is located on 8q22, also known as Astrocyte Elevated Gene-1 (AEG-1) gene, a number of studies show that the the MTDH gene expression regulation in tumor invasion and metastasis chemoresistance, tumor angiogenesis, and aging. In this study, real-time quantitative RT-PCR to detect the peripheral blood of patients with breast cancer the MTDH gene expression level analysis of the relationship between the clinical and pathological features and clinical significance explore MTDH gene can be used as a predictor of breast cancer prognosis. Method: 1, Application spin column extraction of total RNA in peripheral blood of 71 patients with breast cancer and its anti-transcribed into cDNA in real-time fluorescent quantitative reverse transcriptase polymerase chain reaction (Real time quantitative RT-PCR) was used to detect MTDH mRNA expression in breast cancer patients in the study with breast cancer prognostic marker and tumor proliferation-associated factor (PCNA, ki67) compare to investigate the relationship between transfer and prognostic indicators of breast cancer and tumor proliferation and invasion; 2, on the part of tracking and detection of breast cancer patients (n = 33), the the MTDH gene expression changes comparing before and after chemotherapy; follow-up of 71 selected patients with breast cancer after treatment, research analysis of the MTDH gene expression and patient survival correlation. Results: 1, real-time quantitative RT-PCR results show MTDH have a certain amount of expression of genes in the peripheral blood of patients with solid tumors such as breast cancer, in breast fibroid patients and healthy volunteers, no positive expression; 2, 71 patients with breast cancer, 61 cases (81.3%) patients the MTDH gene due to the positive expression, of which 47.5% (29/61) of patients with peripheral blood the MTDH genes expressed at high levels; 3, the peripheral blood of breast cancer patients MTDH The amount of gene expression was positively correlated with lymph node metastasis, MTDH expression of lymph node metastasis (1 ≤ LNM ≤ 3) is about 1.548 times the group without lymph node metastasis; lymph node metastasis (LNM gt; 3) MTDH expression level was about lymph node metastasis group (1 ≤ LNM ≤ 3) 2.085 times about lymph nodes without metastasis 3.227 times; 4 the MTDH gene expression levels in the peripheral blood of breast cancer patients in clinical stage there are differences among the groups (P lt ; 0.05), which Ⅳ patients the MTDH gene expression amount Ⅰ patients 6.774, 3.387 times the phase Ⅱ, the MTDH gene expression in the amount of the Phase III patients is 4.563 times Ⅰ patients; 5, the outer periphery of the breast cancer patients blood the MTDH gene expression amount and the patient's age, ER, PR and HER-2 independent of (P values ??were 0.784,0.164,0.154,0.577 P lt; 0.05); peripheral blood of breast cancer patients MTDH gene expression and tumor proliferation relevant factor PCNA, Ki67 unrelated (P values ??were 0.093,0.110) patients showed positive expression of these two factors have a positive correlation (P = 0.040), suggesting that MTDH gene expression in tumor invasion and metastasis has important role; 7, part MTDH gene positive patients after adjuvant chemotherapy for 2-3 cycles again detected in 14 patients MTDH gene became negative, negative rate of 60.9% (14/23), chemotherapy can significantly reduce the gene expression levels. Conclusion: 1 MTDH gene was detected in the peripheral blood of patients with breast cancer, lung cancer and other solid tumors, suggesting that circulating tumor cells into the bloodstream that micrometastases may be present in the blood is expected to become so MTDH gene markers to determine tumor early metastasis ; 2 in the peripheral blood of patients with breast cancer found in the the MTDH gene quantitative expression and relationship with clinical and pathological features of breast cancer research the MTDH gene quantitative expression correlated with lymph node metastasis and clinical stage were positively correlated, and MTDH gene quantitative expression of the patient The survival time was negatively correlated; MTDH gene in the peripheral blood of breast cancer patients before and after adjuvant chemotherapy quantitatively express significant change, and its discovery the MTDH gene expression in breast cancer patients with poor prognosis and shorter survival. In short, the the MTDH gene expression levels in the peripheral blood of breast cancer patients with poor prognostic factors closely related to tumor markers can be used as an independent predictor of prognosis.
