Abstract


Objective To investigate the intravenous infusion of nitroglycerin (NTG) to intervene 99mTc methoxy isobutyl Clear (99mTc-MIBI) gated single photon emission computed tomography (SPECT) myocardial perfusion imaging and wall motion (WM) and wall motion thickening (WT) in detecting viable myocardium value. Methods 41 cases of patients with myocardial infarction underwent 99mTc-MIBI gated resting scintigraphy and 24-hour intravenous infusion of nitroglycerin standards again intravenous injection of 99mTc-MIBI line resting gated myocardial SPECT and comparative analysis section regional myocardial perfusion and regional wall motion (Wall Motion, WM) and regional wall motion thickening (the Wall Thickening WT) change. Results myocardial segments perfusion improvement: 41 patients with a total of 820 myocardial segments, of which 491 myocardial perfusion abnormalities segment, the segment of radioactive reduce 162, radioactive seriously reduce the segment 157, radiation defect section 172 segment; NTG intervention 99mTc-MIBI imaging perfusion abnormalities segments were detected in 384 (46.83%), of which 151 segments of radioactive reduce radioactive seriously reduce the segment 143, the radiation defect segments 90, NTG intervention, completely refill 107 segments 21.79%, and then filled with 82 segments of 16.70%, the overall improvement in segment total abnormal segment of the overall improvement in segment accounting for 38.49% of the total abnormal segment [( 107 82) / 491]; segment 2.NTG before and after the intervention of the perfusion image display value of myocardial perfusion (tracer relative uptake) with WM, the change in weight of a better correlation, respectively r = 0.53, r = 0.49 and r = 0.74, r = 0.72; compare NTG before and after the intervention of global and local changes in the WM, p gt; 0.05 was not statistically significant, no significant improvement in WM; NTG before and after the intervention global and local WT p lt; 0.001 statistically significant, WT significantly improved, the WT is more valuable in the reaction of myocardial contractile function. Myocardial perfusion image conclusion 1.NTG intervention of 99mTc-MIBI SPECT gated myocardial perfusion imaging quantitative analysis and evaluation of myocardial infarction, infarct location, size and number of the share percentage and viable myocardium; 2. SPECT gated gated myocardial perfusion imaging for wall motion (WM) and wall motion thickening (WT), can improve the survival of patients with myocardial infarction accuracy and detection rate of myocardial cells; WM and WT can reflect myocardial activity and myocardial movement, where WT is more valuable; 3.NTG intervention of 99mTc-MIBI SPECT gated myocardial perfusion imaging myocardial perfusion images with wall motion (WM) and wall motion thickening (WT) and so on the gated information can be quantitatively and comprehensive analysis and evaluation of myocardial viability in patients with coronary heart disease and the local and global myocardial motion function to determine which patients from the coronary revascularization benefit; 4.NTG intervention 99mTc- MIBI SPECT gated myocardial perfusion imaging operation safe, convenient, noninvasive imaging can be obtained without increasing the patient's medical expenses on the basis of perfusion images and WM and WT gated coronary heart disease patients coronary revascularization and risk stratification provides an important guiding role.
