Preface severe acute pancreatitis (SAP) has a rapid onset, severe illness, disease development, traditional therapeutic effect of poor characteristics, current treatment, the mortality rate is still as high as 22.7%, so looking for the ideal therapy has become the treatment The key to this disease. Domestic and foreign scholars in recent years, the application of low-molecular-weight heparin (LMWH) in the treatment of experimental and clinical aspects of SAP conducted a series of in-depth study, found the LMWH to improve microcirculation and inhibit the inflammatory response in rats with SAP. Past studies found the Lithospermum regulate immunity, anti-inflammatory, anti-tumor and liver and other role, but comfrey is valid for SAP has not been reported. The present study aimed LMWH and the comfrey combination of SAP serum SOD, MDA, NO content and renal expression of HSP70; combination on whether a synergistic effect in the treatment of SAP. Objective: To observe LMWH and comfrey mortality of rats with SAP, serum SOD, MDA and NO at different time points dynamically change the impact of renal tissue expression of HSP70, and then explore the combination of LMWH and comfrey SAP mechanism of action explore effective measures to reduce the SAP provide a theoretical basis for clinical treatment. Methods: The rats were randomly divided into five groups: (1) sham operation group (group A, only underwent open surgery); (3) SAP (2) severe acute pancreatitis group (group B or model group); (4) SAP comfrey treatment group (Group D or comfrey group); (5) SAP low molecular weight heparin and comfrey combination therapy group (group E or compound of molecular weight heparin treatment group (group C or heparin group); group), (4 groups were made of the SAP model), (n = 18); using sodium taurocholate induced rat model of SAP. The postoperative group C immediately given LMWH (100iU/kg/12h) intramuscular injection; D group immediately given the comfrey orally (3g/kg, 2 times a day); the E group give the joint LMWH and lithospermum treatment; group B was given normal saline control. Respectively, in different postoperative time (12h, 24h, 48h and 72h) in each group anesthesia 6 rats, blood detection of serum SOD, MDA and NO content and microscope 24h and 72h renal tissue expression of HSP70 was also compared after 5 rats 24h and 3d mortality. All experimental data are presented as mean ± standard deviation (x ± s) that the measurement data with the F-test, the mortality rate x2 test. Results: (1) mortality observed: A group of rats after all survived, the mortality rate was 0%; the 24h mortality: the B group to 55.56% in group C to 11.11%, D group to 27.78%, 11.11 E group %; 3d mortality: the B group of up to 88.89%, C group was 16.67%, D group was 50.00%; the E group remains to 11.11%; C group D group and E group compared with group B difference was not statistically significant significance (P lt; 0.05, P lt; 0.01), a full description of the SAP model has been established; E and C groups compare statistically the difference was not significant (P GT; 0.05), E group compared with group D, statistically The treatment difference was statistically significant (P lt; 0.05). (2) SOD activity: A group comparison difference with group B have significantly (P lt; 0.01); postoperative at each time point C group and E group comparison difference with group B have significantly (P lt; 0.01); D group 48h, 72h and B group differences have significant (P lt; 0.05, P lt; 0.01), E group and D group after surgery at each time point comparison difference significant (P lt; 0.01 or P lt; 0.05) E group compared with C group difference was not statistically significant (P gt; 0.05) (3) MDA content: 12h each group and B group comparison difference was no significant (P gt; 0.05); 24h, 48h and 72h, group of group C and E and B group comparison difference there is significant (P lt; 0.01) 48h and 72h, D group and B group, the difference was significant (P lt; 0.05), after each time point E group and C group and D group, the difference was not statistically significant (P GT; 0.05) (4) NO content: 12h each group and B group difference was significant (P gt; 0.05); 24ha compare differences and group B there was significant (P lt; 0.01), no significant (P for the remaining three groups differences gt; 0.05); 48h and 72h groups with Group B compare differences have significant (P lt; 0.01, P lt; 0.05), differences in postoperative each time point E group and C group and D group comparison was significant (P gt; 0.05). (5) renal tissue heat shock protein 70 expression: HSP70 expression in 24h after more than 72 hours, statistically significant difference (P lt; 0.05, P lt; 0.01). Compared with group A, group B in renal tissue HSP70 expression was significantly increased, there is a significant difference (P lt; 0.01). Group C, D group and E group HSP70 expression was significantly decreased, three groups compared with group B, there are significant differences (P lt; 0.01), E group and C group D group groups comparison, no significant difference (P GT ; 0.05) Conclusion low molecular weight heparin and comfrey combination can significantly reduce the mortality of rats with SAP, to improve the activity of SOD, inhibition of MDA, increase the content of NO reduced the SAP renal expression of HSP70, protect renal tissue, significantly improved pancreatic microcirculation, suppress the inflammatory response of the SAP, SAP play a therapeutic role, which may be related to dilate blood vessels and anti-free radical damage effect and maintain cell membrane structure stability. Single low-molecular-weight heparin, comfrey treatment of SAP have a certain effect, but the combination of comfrey as effective as low-molecular-weight heparin, low molecular weight heparin and comfrey SAP treatment, but no significant synergy in combination therapy.
