Abstract


Back groundIn recent years, the public health emergencies, such as Biological terrorist attack, SARS, Avian Influenza, and Influenza A (H1N1), outbreaks frequently. So many countries put a lot of funds in preparedness and responding. After SARS, China has increased investment in public health system, but there has no theoretical system and the support mechanism on how to use the emergency expense. Therefore, a research on the expense support is necessary to response effectively to the public health emergencies.ObjectiveSummarize the successful model of expense support to public health emergencies overseas. Analysis the major problems of expense support to public health emergencies in domestic. Research the characteristics, influencing factors, financing source, allocated ways, and segregation of duties of the emergency expense support. And predict the expense to public health emergency response by a simulation model according to the situation in shanghai.MethodsThis research use the Literature review, Focus group, expert advice and other methods in management science to construct the theoretical system of emergency expense support. Then we have used the Regression analysis and Stochastic inventory model to predict the routine reserve expense and emergency reserve expense.Results1 Clarified the concept, characteristics and influence factors of the expense support to public health emergencies.2 Summarizes the raising, operation, distributing and supervising mechanisms of expense support to public health emergencies.3 Proposed to establish a diversified financing sources model, and implement a multi-year accumulation reserve funding.4 A multi-level expense support plan to public health emergencies should be established in shanghai.ConclusionThe Emergency expense support should play an important role to response public health emergencies. Currently, the management of standardized and scientific level is poor in china, so leading to emergency preparedness can not achieve the targets by financial constraints. In the developed countries, the management philosophy of expense support to public health emergencies has shift to emphasize prevention. We should clear financial responsibilities at all levels, increase the preventive financial investments, and specify the object and standard of emergency expense support. In order to protect the public health, we should establish a mechanism of emergency expense support with responsive, efficient operation, and effective support.
