Abstract


The purpose of using ultrasound biomicroscopy (Ultrasound Biomicroscopy, UBM) observed line glass body resection combined with cataract extraction and intraocular lens implantation iris, pupil anatomical changes and clinical intervention effect. The method of postoperative follow-up of 50 simple cataract surgery patients in the combined surgery of the anterior and posterior sections of the success of our hospital 50 patients 40 cases 44 December 2006 12-2007 application UBM preoperative and postoperative a week later this month, after January 1 every two weeks to review the continuous observation of three months. Focus on eye surgery preoperative postoperative anterior chamber depth, angle opening distance, iris, pupil changes were measured and recorded, and 2 patients comparative analysis. About four weeks after the synechia eye needle dial dissection. The eye surgery Tracking observed for 3 months or more. 1) joint operation group of 50 cases of retinal anatomic reduction patients; preoperative and postoperative one week in January and March of deep anterior chamber axis (anterior chamber distance, ACD) were 2305 ± 289μm, 3370 ± 478μm, 3455 ± 456μm, 3487 ± 352μm, compared to postoperative and preoperative ACD significant difference, P lt; 0.05; preoperative, postoperative 1 week in January and March angle opening distance (angle opening distance, AOD 500 ) were 0.427 ± 0.087μm, 0.578 ± 0.093μm, 0.584 ± 0.100μm, 0.567 ± 0.101μm, compared to postoperative and preoperative AOD significant difference, P lt; 0.05 2) Joint surgery 42 (84%), group iris sticky adhesions occur in the postoperative 1-3 weeks period of time; the adhesion sites above 13 accounted for 30.9%, the temporal side 19 accounted for 45.2%, the nasal 8 accounted for 19%, below 2 accounted for 4.9%; iris adhesions line separation postoperative cases no longer occur. Three cases occurred pupillary capture. The simple cataracts group of 40 patients 44 no case occurred iris adhesions, IOL are all located in the capsular bag. 3) to fully dilate the combined surgery group preoperative pupil size was 7.9 ± 1.2mm, 3 months after surgery was 4.2 ± 1.5mm, postoperative and preoperative pupil diameter compared significant difference, (p lt; 0.05) simple cataract group pupil diameter after preoperative was no significant difference (P gt; 0.05). Joint operation group and cataract group were fully expanded pupil size was 4.2 ± 1.5mm and 7.8 ± 1.2mm, the two groups were significantly different (P lt; 0.05). Conclusion vitrectomy combined with cataract extraction and intraocular lens implantation compared with simple cataract surgery, a very high incidence of posterior synechia, is the most common postoperative anterior segment complications, and postoperative pupil is difficult to expand, more serious combined with postoperative due to inflammatory reaction. Inflammation control underwent iris adhesions pin stripping separation surgery is a safe and effective treatment method.
