Abstract


Objective To explore a relatively ideal method of ear reconstruction surgery. Since microtia and acquired auricular defects are more common diseases, seriously affecting the patient's physical and mental health, social and family to bring a heavy burden, and ear reconstruction surgery is the field of organ and Reconstructive Surgery hardest and most complex surgery world. The previous conventional ear reconstruction surgery completed in phases, treatment length, time-consuming and laborious, and its effect is difficult to make a patient satisfaction. The main subject is to design a relatively ideal way of total ear reconstruction surgery and covering materials, ear reconstruction once the procedure is completed, recycling ear outline clear, realistic shape, auriculocephalic angle remained relatively stable, so that the covering material can fully express the bracket shape to express the fine structure of the normal ear, the helix crus and offal, ear boat with triangular fossa, tragus of the ear and ear screen and concha can clearly apparent. Method 1. Through literature review, right temporoparietal fascia (TPF) morphology and blood supply analysis. Familiar with the size range of TPF, thickness and its connection with the surrounding tissue, the blood supply to understand TPF familiar with the superficial temporal artery (STA) and the adjacent scalp artery anastomosis between the situation, understand its terminal branches of the STA walk the line, branch diameter sizes and probability of occurrence. (2) design a medpor bracket carving methods, can fully express the bracket shape, recycling and ears to express the fine structure of the normal ear, including the ear boat with triangular fossa was interchange, and the helix crus scraps of tragus and tragus, and has a sense of beauty. 3 to top of the superficial temporal artery branch (PB) for the vessel axis temporoparietal fascia design, choose 4 cases using medpor bracket line one ear reconstruction surgery to temporoparietal fascial flap wrapped ears stand, covered skin, choose whole thickness skin, complete ear reconstruction, patients were followed for one year term clinical effects observed. Results 1. Designed medpor bracket reverse modeling method engraving, carving of the stent can fully express the normal appearance of the fine structure of the ear, and has a sense of beauty. 2.4 cases of patients using medpor bracket line one ear reconstruction, reconstruction ear pinna bump appeared, chiseled, ministries clearly structured, three-dimensional sense of strong, shape realistic, long-term effect is good, four cases occurred in 1 patient hematoma, but did not occur stent deformation, stand exposed and other complications. Conclusion temporoparietal fascial flap ear reconstruction surgery is an ideal kind of ear reconstruction surgery. 1. Medpor ears stand stable, easy carving shape, internal porous conducive to tissue ingrowth, and soft tissue with a solid, good histocompatibility non-toxic, is the ideal ear scaffold. 2 temporoparietal fascia vascular constant, the temporoparietal fascial flap flip stent wrapped ear ear reconstruction may well be a more ideal way to reduce the incidence of stent exposed, and can fully show the fine structure of the stent.
