Abstract


Objective: To investigate the bladder diverticulum cancer diagnosis and treatment characteristics.Methods: It’s a retrospective analysis of five cases of the bladder diverticulum cancer clinical data over the past 10 year,including clinical features,diagnosis and treatment of various options. The age group is 53-74 years old,with an average age of 67. Three cases have gross hematuria,one case has a microscopic hematuria,one case hsa the difficulties in urination . Found that bladder diverticulum and tumor intervals: 4 cases at the same time , one case was found in the operation ,three cases of bladder diverticulum were discovered and two cases were found. but found open the bladder diverticulum;3 cases were performed KUB+IVU,and two of themwere found bladder diverticulum and tumor,one case was dissatisfied with the enhancement of bladder diverticulum,no tips diagnosis; three cases were performed CT scan,bladder diverticula and indoor soft tissue block video were found ,All the cancer was enhanced in varying degrees.4 cases were performed cystoscopy,three cases of bladder diverticulum cancer were found and were performed tumor tissue biopsy,one case had small bladder diverticulum neck and the calculuss,so the cystoscopy and biopsy were difficult to be done,it was confirmed by postoperative pathology. All the cases were performed surgery,two cases of preoperative diagnosis were quick frozen tumor tissue biopsy diagnosis. The bladder diverticulum of the wall were thin and had muscular dysplasia. 5 patients were performed partial cystectomy,removal of bladder diverticulum,the tumor the and surrounding the bladder diverticulum wall,two cases were performed ureteral reimplantation in situ at the same time,one case was performed prostatectomy at the same time,and all were given postoperatively bladder irrigation such as hydroxycamptothecin 20mg / times a week,a total of six weeks, half a month intervals,a monthintervals,three monthsintervals,six months intervals.Results: All cases were confirmed by pathology. All the types of cancer in this group were transitional cell carcinoma. Because bladder diverticulum has no muscle,tumor can invade directly from the epithelial tissue to the bladder surrounding tissue,the lack of diverticulum tumor stage,according to the 2002 TNM staging of bladder cancer,it’s lack of T2 ,tumor can invade directly from the T1 (tumor invade the epithelial tissue) to T3 ( tumor invade the bladder surrounding tissue). The clinical manifestation of such tumors is hematuria,early diagnosis is difficult, The partial cystectomy is main treatment.1,3 and 5 year survival rate was 74% ~ 92%,60% ~ 74% and ~ 31% 59%,and the prognosis is closely related to tumor staging.Conclusions1. Bladder diverticulum cancer specifical symptom, the bladder diverticulum cancer early diagnosis is still difficult,relying on the diagnosis of bladder endoscopic biopsy pathology,but we can not ignore other diagnostic methods used.2. Bladder diverticulum cancer treatment is partial cystectomy. appropriate chemotherapy is also necessary.3. Incomplete emptying of the diverticulum should surgery. Rest room should be regularly inspected,superficial bladder cancer by type of resection and intravesical chemotherapy,the treatment of bladder reservations can be cured,so the bladder diverticulum patient follow-up review should attach importance to and improve bladder diverticulum in the early diagnosis of cancer and the therapeutic effect.
