Abstract


Objective: To investigate during anesthesia induction , body mass index and head high 15 ° tilt bits oxygen to nitrogen impact on patient tolerance hypoxia . Methods: Age 21 to 63 years old, good cardio , 62 cases of non - specific disorders anesthesia of intubated patients . First , based on body mass index (BMI) : normal weight ( 18.5 ≤ BMI lt ; 23 kg / m 2 ) in 17 cases , ultra reorganization ( 23 ≤ BMI lt ; 25 kg / m 2 ) 13 cases , 1 ° obese group ( 25 ≤ BMI lt; 30kg / ㎡ ) 15 cases and 2 ° obese group ( BMI ≥ 30 kg/m2 ) 17 cases of four groups . Then randomly divided into the supine group ( 30 cases ) and 15 ° head high tilt -bit group ( 32 cases ) . Conventional intravenous induction of anesthesia , during induction of oxygen flow rate of 10L/min sufficient oxygen to nitrogen four minutes , induced by the end of tracheal intubation continue deoxy observed after intubation , pending pulse oxygen saturation (SPO2) to 90% when access anesthesia machine breathing circuit control respiration . Tolerance to asphyxia safety time limit record ( induction of anesthesia at the end ~ SPO2 decreased to 90 % of the time asphyxia time ) and go and SPO2 nitrogen to 90% three point-in-time acquisition burglary early oxygen arterial blood for blood gas analysis . Results: BMI significantly different patient tolerance to asphyxia safety time limit (DSP-Desaturation Safety Period) , tolerated to asphyxia safety time limit gradually shorten with increasing BMI . 4 study group patients tolerated to asphyxia safety time limit were 546.47 ± 75.20 s ( normal weight ) , 465.77 ± 32.20s ( overweight ), 355.13 ± 44.36s (1 ° obese group ) , 227.53 ± 34.23s ( 2 ° obese group ) , the difference was significant ( P lt; 0.05 ) . Head high 15 ° tilt bit supine oxygen to nitrogen compared , can effectively extend the the patient tolerance asphyxia security time limit of about 70 seconds , withstand asphyxia safety time limit are ± 359.33s 124.42s ( flat 15 ° tilt supine group ) and 430.00s ± 136.63s ( head high bit set ) , the difference between groups was statistically significant ( P lt; 0.05 ) . Conclusion: BMI , patients tolerated hypoxia significantly different potential , its tolerance to hypoxia gradually shortened with increasing BMI ; head high 15 ° tilt bit supine oxygen to nitrogen compared can effectively extend the time patients tolerated hypoxia .
