Abstract


The purpose of the use of multifocal electroretinography (multifocal electroretinogram, mERG) inspection techniques diabetic retinopathy (diabetic retinopathy, the DR) laser treatment of retinal function changes and trends, combined with optical tomography (optical coherence tomography, OCT) understand macular district organizational structure change and the corresponding changes in retinal function, multifocal ERG in the clinical treatment of diabetic retinopathy, which has accumulated experience in the practical application of the inspection techniques in the study of disease. Method: 1. Selected health in the elderly 35 35 the multifocal ERG check and record the results, its value as a normal reference values. Select the Tianjin Medical University Eye Center diagnosed the proliferation of early diabetic retinopathy (preproliferative diabetic retinopathy, pre-PDR) and laser treatment of 30 patients with 40 in July 2006 - December 2007, the application of multifocal electrophysiological tester and optical coherence tomography laser before and after laser 2 days, 1 week, 2 weeks, March mERG and macular OCT examination, and the results were statistically analyzed. 3. Statistical methods: using SPSS11.5 statistical package for data statistical analysis, the variables independent samples t-test, analysis of variance and two-variable correlation analysis of randomized block design, with P <0.05 as statistically difference Significance standards. Results: Compared with normal control group, to proliferation the pre diabetic retinopathy mERG P 1 -wave amplitude in ring 2 - Ring 5 quadrant Ⅱ was significantly decreased (P <0.05), N 1 -wave amplitude in ring 3, ring 4 quadrant Ⅲ was significantly decreased (P <0.05); 1 wave latency quadrant Ⅲ appear in the ring - ring 5 <0.05), N 1 wave latency in 4 quadrant Ⅰ significant delay (P <0.05). Before and after laser periods P 1 -wave amplitude in ring 1, ring 3 - ring 5, the quadrant Ⅰ - Ⅳ significant difference (P <0.01), N 1 wave amplitude in ring 1, ring 5, the quadrant Ⅱ Ⅳ significant differences (P <0.05); P 1 wave latency in the ring - ring 5, the quadrant Ⅰ, Ⅲ significant difference ( P <0.05), N 1 wave latency only in ring 5 there were significant differences (P <0.05). Laser before and after stages of macular center concave thickness significantly with difference (P <0.05), in which the laser after 2 Tian, ??the center of the macula concave retinal thickness 274.85 ± 108.20μm, and preoperative there are significant with difference (P = 0.009), 3 months after foveal thickness before surgery was no significant difference (P = 0.884). 4 laser before and after laser one week foveal thickness corresponding P 1 wave amplitude correlation coefficients were r = -0.775 (P = 0.002) and r = -0.594 (P = 0.025) between was a significant negative correlation. Conclusion: The proliferation of early diabetic retinopathy (pre-PDR) in patients with retinal function was compromised the reaction mERG there are different degrees of reduction. After laser treatment, the laser itself retinal function and retinal autoantibodies damage the ability of the weakened retinal function in the short term a significant decline, followed by a gradual recovery trend, but the long-term observation of the results showed that the retinal function than the laser before significantly reduced, while the laser before and after laser 1 week, foveal thickness corresponding P 1 wave amplitude was significantly negatively correlated, and that the more severe macular edema, P 1 wave amplitude lower. In short, as the emerging visual electrophysiological examination technology, multifocal ERG (mERG) can be recorded in the short period of time the reaction of the local area in the retina, enabling evaluation of the the test wild in retinal function, this feature is also that equally important value in the detection of macular degeneration. This study will mERG applied to the proliferation of early diabetic retinopathy laser treatment of retinal function before and after the determination and the joint OCT observe changes in the the macular cross section of tissue morphology and corresponding retinal function, not only for the macular area to provide a more accurate diagnosis of subtle lesions basis, is also more fully evaluate changes in retinal function. MERG objective, accurate positioning, quantitative record of retinal function, and thus is a very effective examination methods in clinical and theoretical research, the adoption of the check, we can get more information about the changes in visual function and the ability to regularly observing the changes of retinal function, progression of the disease and the effects of the clinics, so mERG in the early observation of disease, follow-up and further treatment of choice has a high value.
