Abstract


The purpose of the establishment of cardiopulmonary bypass (CPB) during continuous perfusion of oxygenated blood through the pulmonary artery and L-arginine, methylprednisolone, Chuan Qiong triazine three drugs, observed pulmonary artery infusion drugs plus mechanical ventilation on lung injury during cardiopulmonary bypass protection role. Methods controlled clinical study of 60 patients undergoing elective conventional mitral valve replacement patients were randomly divided into four groups: control group, continuous pulmonary artery perfusion of oxygenated blood plus mechanical ventilation in the control group on the basis of the remaining groups were added to L-arginine (200mg/kg), methylprednisolone (5mg/kg), containing Chuan Qiong triazine (5mg/kg), n = 15. Establish vital signs monitoring systems, conventional intravenous and inhalation anesthesia, using a single-lumen endotracheal intubation, four groups of patients using conventional the CPB, the whole group of patients, respectively, after the anesthesia, open aortic 1 hour, back ICU0, 6, 12, 24 hours to take the radial artery, the determination of tumor necrosis factor (TNF-α), interleukin-6 (IL-6), interleukin-10 (IL-10) expression. (1) 60 patients, intraoperative and postoperative results were cured smoothly. (2) open aortic dosing plasma TNF-α levels significantly lower than the control group (P <0.05), but in the three groups dosing group: plasma TNF-Chuan Qiong triazine with methylprednisolone group α level is lower than the L-arginine group (P <0.05), no significant difference in, Chuan Qiong triazine with methylprednisolone group TNF-α level; (3) open aorta after dosing plasma IL-6 levels were significantly lower than the control group (P <0.05), but the in dosing group three groups: plasma IL-6 levels lower than the methylprednisolone group (P <0.05), Chuan Qiong hydrochloride and L-arginine group Chuan Qiong triazine plasma IL-6 levels lower than the L-arginine (P <0.05); (4) open aorta after dosing plasma IL-10 levels significantly higher than that in the control group (P <0.05), but in the three groups plus Chuan Qiong hydrochloride and L-arginine group in the drug group: plasma IL-10 levels higher than the methylprednisolone group (P <0.05), Chuan Qiong triazine plasma IL-6 levels higher than that of L-arginine (P <0.05) . Conclusion continuous pulmonary artery perfusion during CPB oxygenated blood to correct obvious CPB during lung ischemia and hypoxia conditions, L-arginine, Chuan Qiong triazine, methylprednisolone can significantly inhibit the synthesis and release of inflammatory cytokines, increased resistance to release of inflammatory cytokines, which prove that the three groups of lung protection dosing group compared with the control group, the comparison of three drugs lung protection: Chuan Qiong triazine role is particularly evident for the lower IL-6 and increased IL-10, L-arginine is superior to methylprednisolone reduce TNF-α, the role of L-arginine is slightly weaker than methylprednisolone.
