Study on Diagnostic Value of Platelet Glycoprotein-specific Autoantibody Detection in Idiopathic Thrombocytopenic Purpura

Abstract


Objective: To compare idiopathic thrombocytopenic purpura (idiopathic thrombocytopenic purpura ITP) in patients with non-immune thrombocytopenia patients and healthy volunteers platelet-specific antibodies GP Ⅱ b / Ⅲ a and GP Ⅰ b / Ⅸ and platelet -associated antibodies (PAIg) expression level , the evaluation of platelet -specific antibodies in the diagnosis of ITP diagnosis . Methods: gold standard diagnosis of idiopathic thrombocytopenic purpura in patients with non-immune thrombocytopenia patients and normal controls , 30 patients by flow cytometry (FCM) detection PAIgG, PAIgA, PAIgM; monoclonal antibodies with improved specificity of platelet antigen capture (MAIPA) to detect anti-platelet antibodies GP Ⅱ b / Ⅲ a and GP Ⅰ b / Ⅸ. Results : PAIg test results in the ITP group, the sensitivity was 80 % instead of immune thrombocytopenia group was 63.3 %, there was no difference in the detection rate PAIg ((x ~ 2 = 2.05, p> 0.05). MAIPA detection of anti- GP Ⅱ b / Ⅲ a and GP Ⅰ b / Ⅸ, in the ITP group two indexes sensitivity were 53.3 % and 30% : In non- immune thrombocytopenia group except one case of GP Ⅰ b / Ⅸ positive , the rest were negative ; None of the positive control group , two groups of two indicators were different detection rates (p <0.05). MAIPA assay of GP Ⅱ b / Ⅲ a and GP Ⅰ b / Ⅸ two indicators for joint experiments ( parallel ) , the detection of idiopathic thrombocytopenic purpura (ITP) patients with a sensitivity of 73.3% and a specificity of 100% , a positive predictive value of 100% . CONCLUSION : PAIg test results , although its sensitivity and specificity are high , but in two group, the detection rate was no difference idiopathic thrombocytopenic purpura (ITP) and non- immune thrombocytopenia patients in the differential diagnosis of little significance . use MAIPA detect anti- GP Ⅱ b / Ⅲ a and GP Ⅰ b / Ⅸ, its sensitivity is far more low, but high specificity , the diagnostic value of large positive results . application joint experiments ( parallel ) method can significantly improve the sensitivity and reduce misdiagnosis. Meanwhile , the index can also be used with idiopathic thrombocytopenic purpura (ITP) and non- immune thrombocytopenia in the differential diagnosis of patients , therefore , improved platelet antigen monoclonal antibody capture technology (MAIPA) platelet membrane glycoprotein -specific autoantibodies can be used as indicators of specific diagnosis of ITP .
