Abstract


Objective: To observe the Discitis histological changes, ultrastructural changes and determination of immunoglobulin (IgG and IgM), collagen and proteoglycan content to study the pathogenesis of the pathological process and the efficacy of antibiotics. Methods: 76 adult New Zealand white rabbits, average weight 2.5kg, 72 rabbits were randomly divided into untreated group and the experimental treatment group (n = 6), established by injection of Staphylococcus aureus by peritoneal cavity intervertebral space lumbar disc infected animal model, experimental treatment group in the postoperative be antibiotic treatment for 1 week, respectively, for 1, 2, 4, 8, and 16 weeks postoperatively, take rabbit lumbar intervertebral discs of 2/3 histological observation, light microscopy and scanning electron microscopy, determination of the adaptive immune globulin (IgG and IgM), collagen and proteoglycans. 4 as a blank control group, the determination of normal immunoglobulin (IgG and IgM), collagen and proteoglycan. Results: Discitis mainly in 1 to 8 weeks of the onset of a disc tissue destruction, infiltration of inflammatory cells in the intervertebral disc of the light microscope, electron microscope, the nucleus pulposus cartilage-like nuclear condensation, cell damage, the lysosomal decomposition; collagen and proteoglycans with the progress of the disease and a significant reduction in the normal control group and intervertebral disc infection untreated group and disc infection treatment group showed a significant difference; immunoglobulin IgG and IgM increased, with a significant difference in the normal control group, but the treatment group and untreated group, no significant difference. Mainly hyperplasia in the pathogenesis of 8 to 16 weeks, light microscope, connective tissue proliferation within the intervertebral space, new bone formation, scanning electron microscope disc cartilage-like cells increased, collagen and proteoglycans remaining at the level of eight weeks, The immunoglobulin gradually restored to normal levels. Significantly reduced inflammation in the treatment group within the intervertebral space, light microscopy and scanning electron microscopy destruction significantly higher than light, collagen and proteoglycans of the non-treatment group compared with the untreated group was significantly higher. Conclusion: of discitis not simple bacterial infectious inflammatory response but bacterial infections and autoimmune damage coexistence complex pathological changes. Discitis in the pathological manifestations of inflammatory destruction and proliferation, mainly in the incidence of 1 to 8 of disc matrix destruction, mainly in the incidence of 8 to 16 weeks within the intervertebral space connective tissue proliferation of new bone formation . In the disease process, especially immunoglobulin IgM abnormal increase, prompted Discitis immune function closely related. Destruction of collagen and proteoglycans, resulting in the loss of function of the intervertebral disc and interbody fusion, antibiotics its treatment.
