Abstract


ObjectiveTo investigate the expression of Micro-fibrin-associated protein 4 in liver tissues of patients with chronic hepatitis B and the concentration of MFAP4 in peripheral blood from the patients above,to analyse the correlation and significance about the two things above separately associated with the activity of liver inflammation and fibrosis staging in the aspect of clinic pathology,in order to provide a theoretical basis for the further study of the mechanism of liver fibrosis and search for new markers of liver fibrosis.MethodsTo Choose 50 cases of patients who suffered chronic hepatitis B and were treated in Liaocheng People’s Hospital from October 2008 to May 2010.All cases are in line with revised diagnostic criteria of the Chinese Medical Association infectious and parasitic credits meeting in Xian in September 2000. All patients were punctured out a bit of liver tissue by liver puncture biopsy for pathological examination,the lager can judge liver inflammatory activity grade(G)and fibrosis staging(S), simultaneously collecting the serum of all the patients above,and collecting 20 cases of serum from healthy examination persons in the same period as control serum. To assessment the degree of liver inflammation grade (G) and fibrosis stage (S) by HE staining and Masson staining, of which 8 cases of G0, G1 10 cases, G2 12 cases, G3 11 cases, G4 9 cases ; S0 10 cases, S1 11 cases, S2 9 cases, S3 9 cases, S4 11 cases. To determine the expression of MFAP4 in liver fibrosis tissue by immunofluorescence histochemistry, and to determine the concentration of serum MFAP4 by enzyme-linked immunosorbent assay double antibody sandwich assay. Statistical analysis was performed by using SPSS 17.0 statistics sofeware package. Results1. The expression of MFAP4 in liver fibrosis: There were 14 cases of liver tissue see positive expression of MFAP4 in all 50 patients. MFAP4 highly expressed in liver fibrosis tissue, mainly in the portal area and extracellular matrix (ECM) deposition area. The stromal cells showed strong expression but no expression in normal liver tissue and in liver parenchymal cells. the positive expression of MFAP4 is closely related to the parts of fibrosis and inflammation. 2. The relationship between the expression of MFAP4 in liver tissue and thegrading of liver inflammation activity: The positive expression cases of MFAP4 in the liver histological grade 0 cases in the G0, G1 3 cases, G2 9 cases, G3 8 cases, G4 7 cases, the positive rate was 0.00%, 30.00%, 75.00%, 72.73% and 77.78%, the difference was significant (P<0.05). The expression of MFAP4 in the liver tissues above showed positively correlation with liver inflammatory G grade.3. The relationship between the expression of MFAP4 in liver tissue and the staging of liver fibrosis degree: The positive expression cases of MFAP4 in the liver histological stage 2 cases in the S0, S1 4 cases, S2 5 cases, S3 7 cases, S4 9 cases, the positive rate was 20.00%, 36.36%, 55.56%, 77.78% and 81.82%, the difference was significant (P<0.05). The expression of MFAP4 in the liver tissues above showed positively correlation with liver fibrosis S stage.4. The relationship between serum concentrations of MFAP4 and the grading of liver inflammation activity: With the heavier activities of liver inflammation, the concentration of MFAP4 in serum gradually increased. Comparing of the normal group and each group of inflammation grade, the results about the determination of MFAP4 indicated that all of the pairwise comparisons between the each pair of adjacent groups had no significant difference,but the comparisons between the each pair of nonadjacent groups were all significantly different(α= 0.05).5. The relationship between serum concentrations of MFAP4 and the staging of liver fibrosis degree: With the heavier degree of liver fibrosis, the concentration of MFAP4 in serum gradually increased. Comparing of the normal group and each group of liver fibrosis staging,the results about the determination of MFAP4 indicate that all of the comparisons between the each pair of adjacent groups had no significant difference, but the comparisons between the each pair of nonadjacent groups showed that only a few results indicated no significant difference(α= 0.05). 6. The concentrations of MFAP4 in serum were positively correlated (r = 0.661, P <0.01) with the degree of liver fibrosis staging, and were positively correlated(r = 0.711, P <0.01) with liver inflammation activity grade. The serum MFAP4 level has non-significant correlation with the expression of MFAP4 in liver tissue (P> 0.05).ConcIusions1. The study found that in normal liver tissue MFAP4 was not expressed but in liver fibrosis tissue was strongly expressed, The expression of MFAP4 may be associated with the occurrence and development of liver fibrosis.2. The expression of MFAP4 mainly in the portal area and extracellular matrix (ECM) deposition area, the cells interstitium showed strong expression, it shows that MFAP4 impact on the liver major by ECM.3. In liver fibrosis tissue, the positive rate of MFAP4 gradually increased with the aggravation of liver fibrosis and inflammation. MFAP4 may play a role in the process of liver fibrosis, it is important to reflect the degree of liver fibrosis.4. The concentrations of MFAP4 in serum gradually increased with the aggravation of liver fibrosis and inflammation, may reflect the occurrence of liver fibrosis in a certain extent, but can not distinguish the degree of liver fibrosis staging accurately. It is well correlated with liver fibrosis staging and liver inflammation activity grade, and can indirectly reflect liver damage degree of chronic active hepatitis, and can offer important reference for diagnosis and prognosis judgement.5. The concentrations of MFAP4 in serum have no correlation with the expression of MFAP4 in liver tissues.6. MFAP4 is a new liver fibrosis biomarkers, may be a indicator that can early diagnose liver fibrosis and reflect the degree of the progress of disease.
