Objective To investigate ordinary endoscopic white vinegar dyed the joint MUC2 and CDX2 detection of on the diagnostic value of gastric intestinal metaplasia. Methods 139 cases of ordinary endoscopic diagnosis of atrophic gastritis, some superficial gastritis and gastric suspicious cases, application of 1.5% white vinegar sprayed the lower part of the gastric antrum and body of stomach mucosa, observed after mucosal changes of color and shape, and in its change biopsy for pathological examination and in some cases the line MUC2 and CDX2 immunohistochemical detection of the most obvious site, did not change significantly cases routinely take a biopsy testing. Results (1) 82 cases of chronic atrophic gastritis cases albino 55 cases, 27 cases of obvious chronic superficial gastritis 52 cases, obvious albino five cases, 47 cases; indistinct suspicious five cases of gastric cancer, which The four cases albino obvious. (2) white vinegar staining combined pathological examination prompts: ① ordinary endoscopic diagnosis of chronic superficial gastritis superficial gastritis and pathological diagnosis coincidence rate was 82.7%, while the the atrophic gastritis compliance rate was only 67.0%; ② The 56 patients with stomach mucosal intestinal metaplasia cases of 50 cases with obvious albino, with intestinal metaplasia in gastric mucosa the albino cases significantly higher than superficial gastritis and atrophic gastritis group, a statistically significant difference; ③ associated with gastric intestinal metaplasia cases of gastric pits Type IV changes mainly two cases for type Ⅲ changes, atrophic gastritis mainly type IV change, no statistically significant difference between them. Superficial gastritis with intestinal metaplasia of gastric pits mainly morphological change type II gastric cancer by changing the main type Ⅵ. (2) The results of immunohistochemistry: (1) albino obvious group of CDX2 expression of MUC2 positive expression rate higher than the albino obvious group P lt; 0.05, significant difference; ② associated with gastric intestinal group of CDX2 expression of MUC2 positive expression rate in not associated with gastric intestinal metaplasia, atrophic gastritis and superficial gastritis group, P lt; 0.05, the difference was statistically significant. ③ CDX2 combination of white vinegar and / or MUC2 positive staining the gastric mucosa albino gastric intestinal metaplasia, the diagnosis rate of 97.1%, higher than 83.4% of the the conventional HE pathological combined albino situation of the rate of diagnosis of gastric intestinal metaplasia. Conclusion (1) endoscopic white vinegar staining the the gastric mucosa albino and gastric the foveolar type Ⅳ change is gastric intestinal metaplasia or dysplasia performance, consistent with MUC2, CDX2 expression; (2) white vinegar stained ordinary endoscopic stomach mucosa bleaching can be used as the basis for the diagnosis of gastric intestinal metaplasia; (3) United MUC2, CDX2 immunohistochemical detection can improve the diagnosis rate of gastric intestinal metaplasia.
