Abstract


Objective: Long-term total parenteral nutrition(TPN) can lead to liver-related complications, the light spectrum only showed elevated liver enzymes, severe liver as hepatic steatosis, cholestasis, cholelithiasis, cholecystitis, and even can quickly develop into liver cirrhosis and liver failure. but its specific to the pathogenesis is still unclear. Toll-like receptors are an important component of innate immunity, and Recognize specific pathogen associated molecular patterns, play an important role in mediating the immune response, and research shows that TLRs plays a key role in the liverimmune injury. In this study, the expression of TOLL-like receptor are detected by RT-PCR,to clear the the mechanism approach of Toll-like receptors with TPN-related liver complications. In order to provide laboratory evidence for clinical prevention and treatment of TPN-related of liver complications.Materials and Methods: 24 specific pathogen free male SD rats were randomly divided into 4 groups : NS, TPN ,1g fish oil group, 3g fish oil group. They were fed commercial rat food, tap water, to maintain 12:12 hours of day and night cycle, temperature is maintained at about 22℃, relative humidity 40% 60%. A silicone catheter was sterilely inserted into the superior vena cava through the right jugular vein under general anesthesia. All rats were allowed food and water for the first two days after surgery to recover from the stress of surgery. From the third day, they received infusion through a multi-channel pump for a whole week. The NS group received saline and rat food as well as water at the same time; the TPN group received TPN (see Table 1 for formula); the fish oil group received fish oil supplement; no food or water was given to the TPN and fish oil groups. The infusion for each group was 20ml for Day 1, 40ml for Day 2, and 60ml daily from Day 3 to Day 7. After the upper treatment, all rats were killed quickly through decollation for liver tissues. PCR array was used to analyze the differential expressions of TLR2,3,4,5,7,9,Myd88. Results: PCR results showed that TLR2, 3, 4,5,7,9,Myd88 in rat liver tissue were expressed. Total parenteral nutrition after liver TLRs2,3,4,5,7,9, Myd88 expression levels compared with NS control group significantly increased; the fish oil group liver TLRs2, 3,4,5,7,9, Myd88 expression levels were significantly lower than the TPN group. Differences were statistically significant (P <0.05). 1g fish oil group can protect the liver immunity.Conclusion: The expression of liver TLRs increased in rat with total parenteral nutrition, this change may lead to disorder in the immune function of liver, and fish oil as a specific immune nutrients has a protective effect to the immune injury caused by total parenteral nutrition. Fish oil dose 1g.kg-1.d-1 can play a role in the protective effect.
