Abstract


The purpose of a good tissue matching condition of long-term survival of kidney transplant patients and postoperative recovery of renal function , panel reactive antibody (PRA) value increased more direct impact on transplant kidney survival , detection of renal transplant patients undergoing before and after the PRA, in particular, is especially important to determine the nature of the highly sensitive transplant recipients in vivo antibody . The experiment by PRA detection of renal transplant recipients HLA humoral sensitization , screening a suitable donor , and the changes in the early diagnosis of acute rejection by the PRA . The antigen plate using Laimu De detect 269 cases of patients with panel reactive antibody ( PRA ) before March 2000 , preoperative tested once a week March 2000 after preoperative detection time , 1,3,5 days after detection postoperative week in January tested once in six months once a month . The test results analysis , by Lai Mude cell plate analysis software . PRA lt; 10% without allergens , 10 % lt ; PRA lt ; 50 % mild sensitization PRA gt ; 50 % for highly sensitized . Comprehensive evaluation after renal transplantation hyperacute rejection (HAR), acute rejection (AR) diagnosis is based on history, clinical symptoms , signs , laboratory parameters , color Doppler B-ultrasound , allograft nephropathy treatment biopsy and anti-rejection treatment effect on . Statistical methods using SPSS software x ~ 2 test on the sample rate . PRA negative group preoperative acute rejection incidence 13.2% 46.2% incidence of acute rejection , PRA positive preoperative group , the difference is statistically significant ( P lt; 0.001 ) . March 2000 after the kidney transplant recipients detection PRA152 cases , the negative postoperative negative PRA preoperative acute rejection rate of 9.5% the negative positive after preoperative acute rejection rate increased significantly, by up to 60% , both of there were significant difference ( P lt; 0.001 ) . PRA positive , 26 cases of transplant recipients again 11 cases , accounting for 42% of the initial transplant PRA positive rate of 4 % . Conclusion With the popularity and development of kidney transplantation , the increase in pre - sensitized renal transplant recipients have become important factors that affect the success of kidney transplantation .
