Abstract


Introduction pancreas and periampullary cancer is one of the highest death rate of malignant disease , cancer mortality in the world fifth . Although postoperative survival rate is relatively low , surgical treatment is still the most effective method of treatment . 1898 Codivilla the head of the pancreas and duodenum gastrectomy , creating a precedent for surgery for pancreatic cancer . After thereafter the Whipple pancreaticoduodenectomy resection ( PD pancreaficoduodenectomy ) , Watson and Long-mire pylorus-preserving PD (PPPD, pylorus-preserving-PD) are two major surgical procedures . PPPD has become the main surgical treatment of chronic pancreatitis and ampullary cancer . With the level of the whole of the surgery , postoperative complications is still high , PD still pancreas and periampullary tumor is a safe and appropriate conventional surgery . Four common complication of PD postoperative pancreatic fistula , gastrointestinal or intra-abdominal hemorrhage , abdominal infection , delayed gastric emptying . The first three fatal complication rate is very low . Opposite the incidence of delayed gastric emptying high and a significant increase in the length of stay and hospital costs , lower quality of life . Clinicians must improve surgical skills to reduce postoperative complications and improve the quality of life after surgery . Therefore, to study the mechanism of PD delayed gastric emptying of great clinical significance and value of its prevention and treatment . In this study, the dynamic detection of PD motilin and gastrin , and to explore the motilin and gastrin clinical significance of the dynamic changes of perioperative clinical improve PD postoperative gastrointestinal function and treatment of gastric emptying provide a theoretical basis the delay ( DGE , delayed gastricemptying , ) . MATERIALS AND METHODS We studied 53 patients ( 28 men , 25 females ) , aged 27 to 79 ( 54.60 ± 12.49 years) . Two cases of perioperative death removed from the scope of the study . 51 cases of cancer of the pancreas and periampullary divided into standard PD ( SPD standard PD ) , pppd , common bile duct anastomosis ( Roux - en - y ) , a prospective study . 40 healthy subjects ( 23 males and 17 females ) , age 39 to 75 ( 57.06 ± 9.12 ) years for the control group . 1 hour before using radioimmunoassay technique , postoperative
