Objective: To summarize the clinical materials of the cases underwent endoscopic third ventriculostomy(ETV), especially to explore the influence factor, settlement of ETV and the creation of evaluation system of the effect.Methods: Twenty-five (male 15,female 10) third ventriculostomies and the related complications were analyzed,especially the influence factor and treatment. We also discussed the creation of evaluation system of the effect.Result: Sixteen had perfect effect, five had improvements in the clinical signs , four were failed :two surgical procedures failed,(one because of intraventricular hemorrhage and the other because of the thick of the bottom of intraventricular), two had no effect and received V-P shunts ubsequently. Complication: Four patients had intraventricular hemorrhage , one patients had cerebrospinal fluid fistula and ventricular infection, two subdural fluid collection.Conclusion: ETV is a safe and effective operation method, but ,for the reason to raise the safety and reduce the complication,using a good instrument, selectting the appropriate candidates, grasping operational skill and knowing the complication of the operation well are the key to complete the operation successfully.It is necessary to creat an evaluation system of the effect.
