The ProSeal laryngeal mask airway (PLMA) is a modified laryngeal mask airway which can improve the safety and applicability by isolating alimentary tract from respiratory tract effectively. The PLMA is safer and has more indications in clinical practice compared with the standard laryngeal mask airway (SLMA) . This study was to observe the maneuverability of the PLMA, effects of its insertion on hemodynamics and its availablity during mechanical ventilation by comparing with the SLMA and own control of the PLMA. In addition, the effects of cricoid pressure manipulation on insertion and ventilation function of the PLMA, and the effects of head flexion on ventilation function and anatomical position of the PLMA were also assessed. This study consists of three parts as follows:Part One:Comparsion of the ProSeal Laryngeal Mask Airway and the Standard Laryngeal Mask Airway in General Anesthetized PatientsMethods: 60 adult patients, ASA physical status I-II, scheduled for elective plastic surgery were randomly allocated to either the PLMA group or the SLMA group. After standard intravenous anesthesia induction, the PLMA or SLMA was inserted using the routine techniques, respectively. The intracuff pressure was set at 60 cmH2O. Noninvasive SBP, DBP and HR were recorded before and after anesthetic induction, at insertion and 5 minutes after insertion with 1 minute interval. In the PLMA group, gastric tube placement through the PLMA was assessed. Respiratory and airway complications in both groups were recorded.Results: All patients in the PLMA group, compared with 27 patients in the SLMA group, achieved satisfactory lung ventilation at the first attempt, but the duration of insertion was significantly longer in the PLMA group than the SLMA group.
