Abstract


Background:Chronic dermatitis represents a group of chronic, relapsing, and inflammatory dermatoses histopathologically characterized by moderate or severe epidermal hyperplasia and inflammatory infiltration in upper dermis. Chronic eczema and lichen simplex chronicus are both common and typical chronic dermatitides with irresistible pruritus, infiltrated plaques or lichenified lesions. Topical glucocorticoid therapy is mainly a routine therapeutic modality for the treatment of chronic dermatitides. Tachyphylaxis and resistance to topical glucocorticoid agents often occur during the treatment of chronic dermatitides. However, the mechanisms of tachyphylaxis and resistance to topical glucocorticoid therapy are unknown.Glucocorticoids (GCs) fulfill multiple biological and pharmacological functions via binding to glucocorticoid receptor (GR). GR and nuclear factorκB (NF-κB) are both important regulatory factors, with entirely opposite effects, involved in immune response and inflammation. NF-κB plays a role in activating transcription of many cytokines and mediators associated with immune response or inflammation while GR may inhibit gene expression of many cytokines and mediators activated by NF-κB. Moreover, TNF-αand IL-1βare both important inflammatory cytokines actively participating in pathophysiological process of skin inflammation via activating their intracellular signaling pathway. Whether the expressions of GR, NF-κB, TNF-αand IL-1βare changed and whether their expressions are associated with tachyphylaxis and resistance to topical glucocorticoid therapy in chronic dermatitides like chronic eczema and lichen simplex chronicus remain unknown up till now.Objective:To investigate expression of GRα, NF-κB, TNF-αand IL-1βin chronic dermatitides for an insight into the physiopathologic features in skin lesions from chronic dermatitides, and for helpful clues to improve therapeutic efficacy of topical glucocorticoid agents in treatment of chronic dermatitides.
