Abstract


Background:The mechanism of severe acute pancreatitis is very complicated and there are several received theories as follows:1. trypsinization; 2. cell factors and inflamatory mediators; 3. microcirculation failure; 4. second attact; 5.excessive activation of leucocyte. Through injuring pancreatic tissue and cell, various etiological factors result in activation of pancreatic enzyme which digest autologous tissue and inflammatory cell infiltration which produce large inflamatory mediators. Then vaso- active factors induce microcirculation failure, while PAF increases permeability of intestine wall and impairs barrier function of epithelium by which bacteria in intestinal tract enter in lymphonodi mesenterici、abdominal cavity and blood circulation.Besides cellular immune function breakdown.So endotoxemia take place, which activiate macrophage and cause cytokinemia.Leucocyte exudate and assemble in organs deponding on attraction of chemotatic factors and mediation of series of adhesion molecules, then release massive acidic hydrolyase、elastin enzyme and various oxygen free radicals and induce multiple organ failure (MOF). Therefore it’s very important to inhibit assemblage and excessive activiation of leucocyte in therapy.Now reseach find that endothelin is not only the important factor in microcirculation failure ,but also correlative to inflammation mediated by leucocyte.Endothelin-Receptor Antagonist can not only correct haemodynamics changes、stabilize capillary permeability、increase functional capillary density(FCD) but also inhibite leucocyte rolling which mediate by P-selectin and E-selectin espressed on the surface of vascular endothelial cell.As the premise leucocyte rolling inhibited can play an important role in inhibiting leucocyte exudating and assembling. This study is designed to investigate the effects of ET-RA to P-selectin and E-selectin expression in SAP animal model and explore the role of ET-RA in inhibiting leucocyte rolling .Methods: Sixty- four SD rats were assigned to 3 groups randomly , including normal control , SAP, SAP plus BQ123(a specific Endothelin A Receptor Antagonist). A rat model of SAP was induced by the intrapancreatic injections of 5% sodium taurocholate with a dosage of 2.5ml/kg .BQ123 injected subcutaneously at a dose of 12.5ug/kg per 6h at 6 h
