Abstract

The purpose of application of three-dimensional conformal radiation therapy technology to reduce cervical cancer after radical radiotherapy in patients with small bowel, rectum, bladder exposed volume. Method 1 dimensional conformal radiation treatment planning systems analysis of the First Hospital of Lanzhou University 2005-02-2006-12 admitted 30 cases of cervical cancer after radical needs radiotherapy in patients with cervical cancer, pelvic irradiation field, to establish the dose-volume histograms and metering The parameters compare patients before and after every wear traditional radiation field treatment plan and fieldwork three-dimensional conformal treatment program. First Hospital of Lanzhou University 2005-02-2006-12 treated 60 cases of cervical cancer after surgery were randomly divided into two groups, were using three-dimensional conformal radiation therapy planning techniques and conventional radiotherapy techniques for postoperative radiotherapy observation radiotherapy complications and calculations survival effects were compared postoperative radiotherapy. Results traditional techniques and conformal techniques mean tumor control probability respectively 0.60% and 0.61%, accept the prescription dose of ≥ 95% of the small intestine, rectum, bladder volume before and after the technology wear the traditional treatment of the radiation field, 40 percent (32 - 70), 99% (86.5-100), 98% (97.2-100), in the three-dimensional conformal treatment technology for 22% (14.5-25.5), 24% (18.1-29.2), 62% (59.1-65.3) (P <0.001 t test). Application of conformal treatment technology, the average normal tissue complication probability decreased from 0.11 to 0.03. Exposed conformal technology in the kidney slight rise conformal compared with the traditional plan the rectosigmoid average minimum dose (D95) from 48.29GY reduce to 4.51GY. 2 conformal group rectum reaction rates of 35.5% the traditional technology group, 61%, two groups contrast the difference was statistically significant (P <0.05), bladder reaction is relatively small, multi-urethral irritation conformal technique 15%, the traditional technology group, 38.7%, two groups contrast the difference was statistically significant (P <0.05). Long-term complications light to moderate radiation proctitis, conformal group in four cases (13.3%), traditional group of 11 patients (36.7%), the difference was statistically significant (P <0.05), radiocystitis inflammation conformal technology group 3 patients (10%), the traditional technology group, 9 cases (30%), the difference was statistically significant (P <0.05). Conclusion 3D conformal treatment technology can significantly reduce the small bowel irradiated volume of the rectum, bladder according to the amount of the reduction also has the advantage, and can increase the dose of the tumor region, thereby enhancing the rate of tumor control without increasing normal tissue toxicity.
