Abstract


BACKGROUND AND PURPOSE atherosclerosis (Atherosclerosis, AS) the main reason for diabetes and coronary heart disease, cerebrovascular disease, and the surrounding vascular disease. Known AS some risk factors such as obesity, high blood pressure, abnormal lipoprotein metabolism in type 2 diabetic population were higher than the non-diabetic population. Leptin and many cytokines are also directly or indirectly involved in the development of AS. Leptin is a protein encoded by the obese (ob) gene product. The basis of studies have shown that leptin induced diabetic atherosclerosis mechanism causing endothelial dysfunction, endothelial cell proliferation and new blood vessel formation induced oxidative stress, thrombosis, promoting the migration and proliferation of vascular smooth muscle cells. In recent years, the general concern of the people of the important role to play in the pathogenesis of diabetic macroangiopathy selectin family. P-selectin-mediated leukocyte and endothelial cells, the interaction between white blood cells and platelets are considered to be one of the initiating factor of atherosclerosis. Serum ferritin is the most abundant protein in body iron, excessive iron storage by peripheral tissue glucose utilization reduced, so that the blood sugar rises further antagonize the action of insulin result. The iron can catalyze the formation of free radicals may be involved in the occurrence of atherosclerosis by promoting lipid oxidation pathways. The study by the determination of type 2 diabetes mellitus (T2DM) patients with serum P-selectin, leptin, ferritin and related metabolic indicators, in order to explore its relationship with type 2 diabetes and macrovascular disease. Populations, and explore a new way of type 2 diabetes and heart and cerebrovascular disease diagnosis, monitoring, condition assessment identified the need for early clinical intervention. Materials and Methods The study used the case-control method, the study 40 cases were divided into type 2 diabetes group and the normal control group. Whether macrovascular disease, type 2 diabetes is divided into vascular disease (MVC) and non-vascular disease (Non-MVC). Enzyme-linked immunosorbent assay (ELISA) fasting serum concentration of P-selectin, leptin concentrations; serum ferritin concentration was measured by radioimmunoassay (RIA), fasting insulin (FINS). Using statistical analysis and regression analysis, correlation analysis between the groups and the group index and macroangiopathy. Results 1. The T2DM serum leptin concentrations were significantly higher than normal control group, serum leptin concentrations higher than the T2DM group in the MVC group Non-MVC group differences were statistically significant (p <0.05). In T2DM group, correlation analysis found that leptin and BMI, LDL-C, P-select factors were positively correlated with FINS was negatively correlated with FBG, TG, TC, HDL-C, the ferritin irrelevant. In T2DM group, correlation analysis, serum ferritin age, BMI, TC, LDL-C significantly correlated with HDL-C was negatively correlated with FBG, TG, P-selectin, leptin, FINS irrelevant . Ferritin as the dependent variable, multiple linear regression analysis, the final LDL-C to enter the equation. T2DM group as a whole, with or without macroangiopathy becomes the dependent variable, ferritin and other risk factors as independent variables, logistic regression analysis, the last TG and LDL-C to enter the equation. Conclusion 1. T2DM group and normal control group found that serum leptin, P-selectin, ferritin concentration was significantly higher: 2. T2DM group the MVC group with the Non-MVC group found significantly elevated serum leptin, P-selectin, ferritin concentration; 3. Serum leptin, P-selectin, ferritin and obesity disorder of glucose metabolism, lipid metabolism disorders are closely related; 4. Serum P-selectin, ferritin, leptin associated with type 2 diabetes, especially the closer relationship between leptin and type 2 diabetes macrovascular disease. May be involved in the formation of atherosclerosis, which is an important risk factor for type 2 diabetic macroangiopathy.
