Abstract


Purpose and background of vascular complications of diabetes (diabetic vascular complications) is one of the important reasons that lead to the death of patients with type 2 diabetes, and more and more research at home and abroad to explore its etiology, and its mechanism of development. Serum ferritin (serum ferritin, SF) is the most abundant protein in the human body iron, to reflect the total amount of iron stored in the body, is to determine an effective indicator of iron deficiency and iron overload. Abroad, many recent scientific studies have shown that there is mutual influence between iron metabolism and type 2 diabetes, iron stores increase indicates that the incidence of type 2 diabetes, iron-induced damage will affect the development of diabetic complications. This issue through observation of type 2 diabetes patients with vascular complications, SF levels and other metabolic markers to explore the relationship between type 2 diabetes patients with vascular complications SF, provide a theoretical basis for the prevention and treatment of diabetic vascular complications. Objects and methods of objects: select the gender, age and diabetes group match of the 100 cases of healthy persons of our hospital diagnosed with type 2 diabetes, 100 cases and medical center. Specimen collection: fasting 8 to 12 hours was measured SF, fasting blood glucose (FBG), fasting insulin (FINS), fasting C-peptide, glycosylated hemoglobin (HbAlc), triglycerides (TG), total cholesterol (TC), low-density lipoprotein cholesterol (LDL), high-density lipoprotein cholesterol (HDL), while collecting 24 hours of patients urine measured 24-hour urinary albumin excretion rate (UAER). Patients with type 2 diabetes underwent color Doppler ultrasound examination of the carotid artery. Insulin sensitivity index (HOMA-IR) = FINS (mIU / L) × FBG (mmol ╱ L) / 22.5. Statistical Methods: All data application SPSS13.0 software for statistical analysis, take p <0.05 was considered significant meaning. Results of the 1.2 diabetes serum SF levels significantly higher than those in the control group (p <0.01). 2. Uncontrolled blood sugar the group SF levels were significantly higher than the basic blood sugar control group (p <0.05) and the control group (p <0.01); basic blood sugar control group SF level with the control group no significant difference. Clinical proteinuria serum SF levels were significantly higher than the microalbuminuria group (p <0.05) and normal albuminuria group (p <0.05); microalbuminuria was significantly higher than normal albuminuria group (p <0.05); Protein the urine each group were significantly higher than those in the control group (p <0.01). The carotid lesion group SF levels significantly higher than the carotid normal group (p <0.05) and the control group (p <0.05). 5. Correlation analysis by Pearson correlation analysis shows that, in patients with type 2 diabetes SF with FINS, UAER/24h, HbAlc, HOMA-IR was a significant positive correlation (p <0.01). Stepwise multiple regression analysis, SF is involved in FINS, UAER/24h, HOMA-IR independent variables. Conclusion SF increases may presage the development of type 2 diabetes and its vascular complications. The high ferritin hyperlipidemia should pay attention to rule out the presence of insulin resistance, impaired glucose tolerance or diabetes. In-depth study of the role of iron metabolism in type 2 diabetes has important significance for further to explicitly type 2 diabetes pathogenesis. Appropriate to reduce iron overload and prevent or delay the development of diabetes and its vascular complications has important clinical value.
