Abstract


Objective: Irritable bowel syndrome (irritable bowel syndrome, IBS), abdominal discomfort or abdominal pain accompanied by abnormal bowel movements mainly a group of intestinal dysfunction syndrome, the lack of obvious morphological and biochemical abnormalities change. Can be divided according to the stool constipation-predominant (IBS-C), diarrhea-predominant (IBS-D), mixed (IBS-M) and Uncertain pathogenic factors and the exact pathogenesis is not yet fully elucidated. The nerve - immune - endocrine Network disorders is a more comprehensive understanding of IBS. The IBS pathophysiology main for gastrointestinal motility disorders, visceral hypersensitivity disorders and neuroendocrine regulation. Gastrointestinal hormones, not only as to the peptidergic neurotransmitters play a role, but also nerves - immunomodulatory mechanisms involved in the pathogenesis of IBS. The gastrointestinal hormone can be divided into two major categories of excitatory and inhibitory, excitatory neurotransmitter serotonin (5-HT), acetylcholine (Ach), P substance (SP), and so on; inhibitory neurotransmitter nitric oxide (NO ), vasoactive intestinal peptide (VIP), calcitonin gene-related peptide (CGRP), these hormones in the gastrointestinal tract and the brain changes affect nerve - immune - endocrine regulation of the network, so that the brain-gut axis disorder, which can lead to a series of changes, such as abnormal gastrointestinal motility, visceral sensitivity changes. Method: 1. Establish a rat model of IBS-D; 2. Rats weight change, diarrhea, calculate the rate of weight change, diarrhea index; 3. Detection hematocrit (HCT), serum electrolytes; 4. Nitrate reductase Law the the serum NO fluorescence method serum level of 5-HT; 5. radioimmunoassay detection of colon tissue VIP, SP content; 6. experimental colon tissue morphology by light microscopy and intestinal mucosal mast cells (MC) change; Results: 1.IBS-D rats obviously persistent diarrhea symptoms, weight downward trend, colon tissue morphology visible mucosal epithelial arrangement than the rule, and no significant hyperemia, edema, ulcers and erosions histopathological changes . Experimental medication can significantly reduce the IBS-D rats diarrhea index (P LT; 0.05), slowing weight loss. The experimental drug has a clear role in antidiarrheal, correctable IBS-D rat blood electrolyte imbalance, improve hematocrit. The experimental drug is high, the middle dose group IBS-D can be effectively raised serum NO ,5-HT levels (P lt; 0.01), to improve gastrointestinal motility disorders. 5 experimental medication, the middle dose group intentionally cut IBS-D the rat colon organizations VIP expression (P lt; 0.05), to participate in the water metabolism regulation. 6. Experimental medication, the middle dose group significantly inhibited the colonic mucosa MC number (P lt; 0.01) and the expression of SP (P lt; 0.05), involved in the regulation of visceral sensory abnormalities. Conclusion: The high lactose feed joint restraint stress-established rat model of IBS-D, with a high incidence of repeatability, can simulate the characteristics of human IBS-D ideal of diarrhea-predominant irritable bowel syndrome model . The experimental medication have clear the antidiarrheal efficacy, and its role in the treatment of IBS-D by regulating neurotransmitter release, and inhibiting the abnormal proliferation of MC, multi-target involved in the regulation of gastrointestinal motility and visceral sensation exception, the combined effects of the multi-level achieved. The experimental medication experimental IBS-D has a dose-dependent effect.
