Abstract

Pregnancy-induced hypertension (pregnancy induced hypertension syndrome PIH) is a common obstetric serious pregnancy complications, the incidence rate of up to 9.4%, an important reason to cause maternal and perinatal deaths, because of its cause is not clear, is still around the focus of the production of medical research. Pregnancy-induced hypertension placenta physiological vascular remodeling obstacles PIH placenta characteristic pathological changes, but also the central link of pregnancy-induced hypertension incidence. The main reason for its nourishing cell dysfunction, hepatocyte growth factor (hepatocyte growth factor HGF) is a pleiotropic regulatory role of cytokines in the newly discovered placenta trophoblast cells function mainly synthesized by the villous stroma cells , the secretion of the receptor c-met in the trophoblast cells and villous vascular endothelial cell surface to a paracrine role in the trophoblastic villi vascular endothelial cells, through the stimulation of cell proliferation, migration, tissue morphology promoting the physiological vascular remodeling occurs formation, the incidence of placenta, mature, and the establishment of a network of villous vessels. The critical role in the physiological processes caused widespread concern. Placenta hypertension, HGF research will help to explore the causes of trophoblast cell dysfunction and secondary pathological changes, in order to clarify the cause. The subject by immunohistochemical methods, combined detection of HGF and c-met in normal early and late pregnancy and pregnancy-induced hypertension expression in the placenta, combined with mechanical indicators of umbilical cord blood flow and villous vascular density studies in normal pregnancy the expression pattern of fluff, pregnancy-induced hypertension correlation between the expression of the placenta and pregnancy-induced hypertension pathological changes. This study is not yet reported. Zhengzhou University graduate thesis PIH placenta HGF and its receptor c-met expression Materials and methods. Subjects divided into three groups (1) normal pregnancy group, 20 cases, for the health of women in early pregnancy, there was no surgical Merger disease. (2) normal pregnant group and 20 cases of healthy late-pregnant women, there was no obstetric complications and medical and surgical complications. (3) PIH group, a total of 58 cases, selected in accordance with the fifth edition of \The patients had no other obstetric complications and medical and surgical complications. ① first group villi taken immediately flow after about 109 409, the first ②, ③ group within 30 minutes after delivery of the placenta of placental maternal surface of the central villi block of approximately lem IcmX lemX of; conventional fixed paraffin-embedded. The immunohistochemical SP (streptavidin / Peroxidas,) method, the detection of HGF and c-met expression; by HE method slice stained count fluff vascular density; ②, ③ group of subjects were in the prenatal week expert color Doppler detection of umbilical blood flow resistance index. Results. Clinical GENERAL INFORMATION light, moderate and severe PIH and normal pregnant group, age, gestational age, baseline blood pressure was no significant difference (P gt; 0 .05), pregnancy-induced hypertension group prenatal systolic blood pressure and diastolic blood pressure compared with the normal pregnant group was significantly higher (P lt; 0 .05), the severe PIH birth weight than normal pregnant group is reduced, the difference was statistically significant (P lt; .05). 2. Normal pregnancy placenta HGF expression in early pregnancy villus HGF expression in the villous stroma, cytotrophoblast and syncytiotrophoblast and decidual cells a small amount of expression. In the placenta of the pregnant group, HGF expression in the villous stroma cells and of fluff vascular endothelial cells, no expression in trophoblast cells and decidual cells. HGF expression in stromal cells in the two groups of fluff compared to late pregnancy group than in the expression of early pregnancy group, the difference was statistically significant (P lt; 0 .05). (3) the PIH group with normal pregnant group placental HGF expression Expression and localization was no difference in the villous stroma cells HGF expression: mild and moderate PIH group compared with normal pregnant group, the difference was not statistically significant (P gt; 0 .05, P gt; 0 .05), severe PIH group than in the normal pregnant group was significantly lower, the difference was statistically significant (P lt; 0 .05). The HGF expression PIH was a negative correlation (r: II 0.319P lt; 0.05). Normal pregnancy placenta c met expression villus c met expressed in cytotrophoblast and syncytiotrophoblast cells and decidual cells, villous vascular endothelial cells and a small amount of coloring. The villous stroma no expression. Late pregnancy the placenta c-met expression sites with early pregnancy, two groups of trophoblast cells c-met expression strength compared with late pregnancy group compared with the early pregnancy group, the difference was statistically significant (P lt; 0.05). 5. PIH group and normal pregnant group placenta c et expression Expression and localization no difference, trophoblast cells c-met expression strength: mild and moderate PIH group compared with the normal pregnant group, the difference was not statistically significant (P gt; 0 .05, P gt; 0.05); severe PIH group than in the normal pregnant group was significantly higher, the difference was statistically significant (P lt; 0 .05). c-met expression was positively correlated (rs = 0 .286 P LT PIH; 0 .05). Interstitial cells of the villi HG Zhengzhou University graduate thesis PIH placenta HGF and its receptor c-met expression F expression to trophoblastic c Inet expression correlation analysis between the two was negatively correlated (rs twenty-one the 0.255P lt; 0 .05). 7.HGF umbilical blood flow resistance index PIH group fetal cardiac end-systolic peak velocity and end-diastolic peak velocity ratio (S / D), vascular resistance index (Rl), blood flow pulsatility index ( Pl) the three umbilical blood flow resistance indicators are high compared with the control group, the difference was statistically significant (P lt; 0 .05). Light, compared to the moderate PIH umbilical blood flow resistance index compared with the control group, the difference was not statistically significant (P gt; 0 .05). Fluff stromal cells expression of HGF strength and S / D, Pl value was negatively correlated (rs 0.376P (0.05, RS = a-0.333P (0.05). 8.HGF villous vascular density placental histopathological examination display: PIH group villous vascular density compared with the normal pregnant group was significantly lower PIH group compared with the normal pregnant group, the difference was statistically significant (P lt; 0.05). villous stroma cells HGF expression and fluff vascular density was positively correlated (rs = 0 .246 P lt; 0 .05) Conclusion 1. early pregnancy and late pregnancy villi, HGF expression in the villi stromal cells and nourish cells, endothelial cells, stromal HGF expression in late pregnancy villi strength is lower than early pregnancy that HGF secretion decreases with gestational age progression, the fluff vascular endothelial cells and expression of trophoblast suggesting that these cells may exist HGF autocrine mechanism; 2. preeclampsia placenta
