Abstract


Object: The ventricular remodeling includes not only the myocardial cell but also the cardiac interstitial collagen. This study was designed to evaluate the extent of myocardial fibrosis in patients with coronary heart disease, or rheumatic heart disease, or dialed cardiomyopathy by measuring serum PINP (procollagen type I), PIIINP (procollagen type III). The role of angiotensionII (AngII), aldosterone (ALD) in myocardial fibrosis was discussed.Methods: Serum PINP, PIIINP, Ang II, and ALD were measured by radioimmuoassay in 76 chronic heart failure patients with coronary heart disease, or rheumatic heart disease, or dialed cardiomyopathy. 20 healthy volunteers served as controls subjects.Result: Serum levels PINP, PIIINP, Ang II, and ALD were significantly increased in all chronic heart failure patients. The levels of PINP, ALD in chronic heart failure patients with dialed cardiomyopathy were higher than palienls with coronary heart disease, or rheumatic heart disease. The ratio of I/III was significantly higher than the healthy volunteersConclusion: 1.There is myocardial collagen remodeling in all chronic heart failure patienls. 2.The angll and ALD play importanl roles on cardiac inlerslilial collagen in postinfarcted remodeling. 3.The myocardial fibrosis of I collagen in dialed cardiomyopathy was more severity than coronary heart disease, or rheumatic heart disease. 4.ACEI has beneficial effecl on interstitial collagen remodeling, inhibil myocardial fibrosis. 5.il is also necessary to treale wilh antifibrosis in chronic heart failure patienls wilh rheumatic heart disease or dialed cardiomyopalhy.
