Purpose in recent years and abroad derived murine model of Schistosoma mansoni infection, a significant dependence on the regulation of cytokine egg granuloma and hepatic fibrosis, but the patients is not clear whether these regulatory applies equally to Schistosoma japonicum infection. Infected persons living environment, including exposure to contaminated water several times more than a decade, but only a small part of the talent will be developed into a severe liver fibrosis. The study assessed chronic Schistosoma japonicum serum Th1-type (IFN-γ, TNF-α) and Th2 (IL-10, IL-13) cytokine levels, trying to reveal the susceptibility of advanced fibrosis cytokine levels relationship, to explore the relationship of these cytokines in the process of Schistosoma japonicum liver fibrosis. The methods subjects were from Schistosoma japonicum epidemic affected areas of Anhui Province, east county career for farmers and fishermen, have more than ten years history of exposure to contaminated water, by the Kato-Katz method Feces Schistosoma japonicum eggs positive 358 patients with advanced schistosome patients. 83 patients with hepatitis B surface antigen positive (23.18%). All patients had B on liver fibrosis assessment grading assess the early stage of liver fibrosis in 101 patients (28.2%), medium-term liver fibrosis, a total of 206 patients (57.5%) of 51 cases of advanced liver fibrosis (14.2 %). Randomly selected 39 cases of patients with early liver fibrosis in hepatitis B surface antigen-negative patients, 7 males and 32 females, aged 13 to 68 years, with an average age of 40.69 ± 10.31 years. Advanced liver fibrosis in 29 patients, 11 males and 18 females, aged 35 to 74 years, with an average age of 57.66 ± 10.59 years. Detection of early and advanced fibrosis patients, serum IL-10, IL-13, IFN-γ, TNF-α, HA, LN levels. The control group for the hospital 20 cases from non-infected healthy persons. Results in 358 patients with advanced schistosomiasis patients with hepatitis B infection rates as high as 23.18%, much higher than the national average in rural areas (10.2%). Schistosome patients with hepatitis B infection compared with uninfected persons, the average age of onset of the early stage of liver fibrosis did not advance; advanced fibrosis patients constitute more than or less. Female patients with advanced fibrosis incidence before age 60 is lower than male patients, but after the age of 60 than men. Cytokine levels detected: Schistosoma japonicum liver fibrosis in patients with serum IL-10 average of 5.73 ± 10.32pg/ml, significantly higher than the normal population, IL-10 levels (1.55 ± 0.23 pg / ml), P lt; 0.01 ; patients with serum IL-13 levels (17.69 ± 10.09 pg / ml) than the normal population (11.05 ± 9.65 pg / ml), P lt; 0.05; patients with serum IFN-γ levels (24.09 ± 40.05 pg / ml) and TNF- α levels (4.76 ± 4.41 pg / ml) IFN-γ levels (1.72 ± 0.46 pg / ml, P lt; 0.01) and TNF-α levels (1.52 ± 1.15 pg / ml, P lt; 0.01) higher than the normal population. Schistosoma japonicum advanced liver fibrosis patients of serum IL-10 levels (9.80 ± 14.68 pg / ml) than early liver fibrosis serum IL-10 levels (2.71 ± 2.65 pg / ml), P lt; 0.05; advanced liver the fibrosis serum IL-13 levels (23.52 ± 11.87 pg / ml) was significantly higher than the early stage of liver fibrosis patients (13.35 ± 5.51 pg / ml), P lt; 0.01; advanced liver fibrosis serum IFN-γ levels (12.90 ± 16.08 pg / ml) lower than that of the early stage of liver fibrosis patients (32.42 ± 49.71 pg / ml), P lt; 0.05; (5.08 ± 4.85 pg / ml) of the patients with advanced liver fibrosis serum levels of TNF-α level in the early stage of liver fibrosis no statistically significant differences in serum TNF-α levels (4.53 ± 4.10 pg / ml). The early stage of liver fibrosis in patients with 3,60-year-old female IL-10 levels (3.13 ± 3.15 pg / ml) than men (1.74 ± 0.36 pg / ml), P lt; 0.05. Men and women with advanced liver fibrosis in patients with IL-10, IL-13, IFN-γ, TNF-α level difference is not. 4, patients with advanced liver fibrosis serum HA levels (273.42 ± 216.54ng/ml) and LN (152.48 ± 43.32 ng / ml) was significantly higher than the early stage of liver fibrosis in patients with serum HA levels (48.88 ± 30.96 ng / ml, P lt ; 0.01) and LN (125.05 ± 19.72 ng / ml, P lt; 0.01). Early liver fibrosis in patients with the level of serum HA and LN normal population level the difference was not statistically significant. 5, IL-10 and IL-13 levels there is a positive correlation (r = 0.375, P lt; 0.01). IL-10 and HA levels there is a positive correlation (r = 0.472, P lt; 0.01). IL-13 and HA levels there is a positive correlation (r = 0.424, P lt; 0.01). HA and LN level there is a positive correlation (r = 0.325, P lt; 0.01). Conclusion cytokines in Schistosoma japonicum early and late stages of liver fibrosis in patients with presence of difference. Serum of patients with advanced fibrosis there is a high level of IL-13, TNF-α levels independent of the degree of fibrosis, which support fiber formation pathway and IL-13 related rather than dependent on the TNF-α pathway. 2 high levels of IFN-γ has a protective effect in of Schistosoma japonicum liver fibrosis patients; 3 IL-10 levels in advanced liver fibrosis group, reason needs further study. 4 experiments have not yet been found Schistosoma japonicum infected with hepatitis B patients compared with schistosome infection, advanced liver fibrosis increased incidence of early age of onset. (5) in of Schistosoma japonicum liver fibrosis patients, serum HA levels as the evaluation of liver fibrosis indicators better than LN.
