Allergic rhinitis (AR) is one of the most common allergic respiratory tract diseases, patients with AR have gradually increased with industrial development and environmental pollution. Although AR had been considered to be type I allergy for a long time, cytokine, neurotransmitter and environmental agents were also involved in its episode later. Recent researches have shown that Nerve Growth Factor (NGF) plays a crucial role in allergic rhinitis, in addition to its neurotophic actions. Our previous studies indicated that the level of NGF protein significantly increased in the nasal mucosa of experiment AR compared with that in control group. Based on the results, we use sensitive ELISA technique to further measure the NGF levels in the nasal mucosa of patients with AR. Furthermore, the mRNA expression of NGF was investigated by RT-RCR, which further explored the roles of NGF in the development of AR.ELISA showed that the NGF content in the nasal mucosa of patients with AR (6.03 1.53ng/g wet weight, n=30) was significantly increased compared with that in control group (3.43 1.19ng/g wet weight, n=10) (P<0.001), which was in accordance with the result of animal experiment and further supported the hypothesis that NGF participated in the pathogenesis of AR.RT-PCR showed that the NGF/GAPDH ratio in experiment group (0.42 0.04)was significantly higher than that in control group (0.21 0.01) (P<0.001). The result suggest that the increasing of NGF content in the patients with AR may partly due to the augment of the expression of NGF mRNA.Desloratadine, a new and selective H1-receptor antagonist with anti-inflammatory activity, was used to treat allergic diseases such as allergic rhinitis and urticaria. In order to preliminarily evaluate the safety and efficacy of the domestic desloratadine tablet and recommend its clinic doses, a four-center, randomized, single-blind, parallel-group, active-controlled trial was performed, which confirmed:1. Desloratadine can obviously alleviate patients’ clinic symptoms, especially for sneezing, rhinorrhea and nasal itching, with its effect similar to that of loratadine tablet.2. No serious adverse effects were found. The incidence rate of side effects was about 12.2%. Side effects, mainly including dry mouth followed by drowsiness and dizziness, were usually alleviated spontaneously, which indicated that the patients have good tolerance to desloratadine. Safe indexes of desloratadine were in the normal range and there were not significant difference between desloratadine and loratadine. The results suggested that desloratadine had no toxic effects for heart, lung, kidney and peripheral blood.To explore the effects on the content of NGF in the patients’ nasal discharge and the correlation between the improvement of patients’ symptom and sign and the change of the content of NGF after treated with desloratadine, before and after the experiment, the content of NGF was measured and the correlation between descent index of NGF and that of symptom and sign was analyzed. To exclude the possible affect of the decrease of nasal mucosa permeability, Human serum albumin (HSA), as the criterion of the nasal mucosa permeability, was measured before and after the experiments and was compared. Results showed that there was no significantdifference in HSA before and after experiments, indicating that there was no obvious change in the nasal mucosa permeability before and after experiments.There was no significant difference in the contents of NGF in nasal discharge before the experiments, but there was quite significant difference after the experiments. The results indicate that the content of NGF in nasal discharge in desloratadine group significantly decreases compared with that in control group.Correlation analysis showed that there was positive correlation between descent index of NGF and that of symptom and sign of patients.
