Abstract


ObjectiveTo study the clinical features of dermatomyositis and polymyositis complicated with thyroid dysfunction.MethodsBetween March2008and December2011, dermatomyositis (DM) and polymyositis (PM) was diagnosed in86patients referred to First Affiliated Hospital of Zhejiang University. Clinical and laboratory data including blood test, urine test, erythrocyte sedimentation rate (ESR), liver function, electrolytes, thyroid function, C reactive protein (CRP), immunoglobulin, antibodies, etc. Special examination including electrocardiogram (ECG), computed tomography (CT), etc. were retrospectively analyzed.Results①Abnormalities of thyroid function in86cases of dermatomyositis/polymyositis patients by up to54cases (62.8%). Most patients suffered from euthyroid sick syndrome, up to28cases (51.9%), followed by sub-clinical hypothyroidism, clinical hypothyroidism, subclinical hyperthyroidism, Hashimoto’s thyroiditis. Compared with normal thyroid hormone group, statistically significant reduction of TT3and FT3(p<0.01)②With regard to clinical features, DM/PM patients with thyroid dysfunction were more likely to have higher incidences of fever (P<0.05)③With regard to laboratory tests, DM/PM patients with thyroid dysfunction were more likely to have higher levels of CRP as well as higher incidences of albumin decreased and blood calcium decline (P<0.01)④With regard to organ involvement, DM/PM patients with thyroid dysfunction were more likely to have higher incidences of pleuritis/pleural effusion (P<0.05)Conclusion①Thyroid dysfunction with DM/PM is not rare, most of whom suffered from euthyroid sick syndrome.②DM/PM patients with thyroid dysfunction were more likely to have higher incidences of fever, higher levels of CRP, albumin decreased, blood calcium decline and pleuritis/pleural effusion. For patients with DM/PM should be give routine thyroid function tests.
