Abstract


Photodynamic therapy (PDT) is the new treatment means of using specific wavelengths excited the photosensitizer, make it occur certain photochemical reactions, to produce toxic side effects on surrounding biological. It essence is based on the characteristics of photosensitive drug have different selection for different type target tissue, then use the appropriate wavelength of light and the light dose of irradiation of the target cells or tissue containing photosensitive drug, finishing the photodynamic effect containing the photosensitive drug-induced optical biochemical reactions. It advantage is that it can selectively kill the inflammatory cells or tissues, but have no or little effect on normal tissues.RA is an inflammatory joint disease, whose pathological basis is synovitis, the basic pathological change is mild to moderate proliferative state lining cells in synovial tissue, inflammatory cell infiltrate spreading to the perivascular, the formation of pannus, which is similar to the connective tissue proliferation and activation. The disease in the clinical shows that synovitis in the joint synovial hyperplasia, later it can cause articular cartilage and bone suffered varying degrees of damage, leading to joint does not work.Currently, according to the severity of the RA, we use different treatment means and measures, including general treatment, medication, surgical treatment, but these means have some shortcoming and deficience. So looking for a bad reaction, without surgery, treatment for RA patients, of all ages is one of the hotspots of academics and medical attention.This article, I expound the basic principles of Photodynamic therapy, and the three principal ingredients; then introduce the pathogenesis of RA, and the current treatment; finally, from the experimental angle to start, studying on the photodynamic treatment of RA. In the process of this experiment, we use670nm wavelength light source, photosensitizer zinc phthalocyanine, and PDT for the observation of the animal model of RA, finally analyzed its treatment effect from erology, pathology angle.The main content of the paper are summarized as follows:Chapter one:IntroductionWe described the development of laser and the technology in the medical profession, and the application in clinical medicine; then I review and summary of the origin and development of light photodynamic therapy. Simultaneously, detailed elaborate on the state of development of photodynamic therapy for rheumatoid arthritis.Chapter two:the overview of photodynamic therapyMainly introduces the main mechanism of photodynamic therapy and the process of energy transfer, followed by the three essential components of photodynamic therapy respectively, and each element plays role in the treatment process; next introduces the effects of photodynamic therapy on the biological tissue.Chapter three:rheumatoid arthritisMainly introduce the pathogenesis of RA; at the same time, analysis of its pathological changes from the perspective of pathological; then anslysis the current treatment state:general treatment of early, medium-term drug treatment, advanced surgical treatment, and put forward their own advantages and disadvantages. It is necessary that find a small side effect, traumatic small, no need operation when possible, and use in patients with various ages treatment.Chapter four:the experimental study on the PDT of RAFrom the features and advantages of photodynamic therapy, we analyze the principle of photodynamic therapy on rheumatoid arthritis, use a basic experimental means to verify its efficacy. We analysis its efficacy from the animals everyday life changes, the ankle volume changes, the perspective of cytokine expression and pathological. We contain the conclution:PDT can inhibit the proliferation of the synovial cells of RA animal model, induce the formation of pannus, and inhibit the expression of the cytokines IL-20and sTfR. The experimental results show that PDT is superior to drug therapy, and it provides a better experimental basis for the clinical practice of PDT treatment of RA.
