Abstract


Objective:To analyze the difference between CKD-EPI equation and simplifiedMDRD equation using to evaluate GFR and explore the applicability of CKD-EPIequation in patients with chronic kidney disease(CKD),under the reference standard ofGlomerular filtration rate(sGFR) measured by99mTC-DTPA renal dynamicimaging.Methods:A total of308patients with CKD which were hospitalized and checkedby the method of99mTC-DTPA renal dynamic imaging between January and December2010were selected for the detection of serum creatinine(Scr) level and GFR wereestimated with CKD-EPI equation and simplified MDRD equation separately.To analyzethe correlation between eGFR and sGFR,and the difference the bias,accuracy andprecision of eGFR evaluated by the two equations.Results:Linear regression analysisshowed that the correlation coefficients between eGFR and sGFR of simplified MDRDequation and CKD-EPI equation were0.818and0.839(P=0.000).There was significantdifference between eGFR and sGFR of the two equations(P=0.005).The bias of eGFRevaluated by CKD-EPI equation was lower0.96ml·min-1·(1.73m2)-1than eGFR evaluatedby simplified MDRD equation.eGFR evaluated by CKD-EPI equation of15%,30%,50%accuracy was28.90%,53.57%,75.32%and eGFR evaluated by simplified MDRD equationof15%,30%,50%accuracy was27.27%,54.22%,76.95%.There were no difference ofaccuracy both in the whole and in different stages of the kidney function.In CKD1stage,the bias of simplified MDRD equation is greater. Bland-Altman analysis theconsistency showed that when sGFR≧60ml·min-1·(1.73m2)-1,the bias was smaller,theprecision was higher of CKD-EPI equation.Conclusion:There is significant differencebetween eGFR evaluated by CKD-EPI equation and simplified MDRD equation andsGFR measured by99mTC-DTPA renal dynamic imaging.CKD-EPI equation showescertain advantages in earlier stage.However,there is some limitation in our research,so whether there is difference between simplified MDRD equation and CKD-EPI equationactually needs more studies.
