Objective:To study the diagnostic values of endogenous carbon monoxide on hemolytic disease of the newborn by measuring carboxyhemoglobin (COHb) and end-tidal carbon monoxide (ETCO).Methods:The newborns with jaundice from September2012to December2012in neonatology department of the Children Hospital, Zhejiang University School of Medicine were divided to hemolytic group and non-hemolytic group due to clinical diagnostic criteria. Hemoglobin (HB), reticulocyte(Ret),serum total bilirubin (STB), COHb and ETCO count were measured on the day of admission and were compared between the two groups. The receiver operating characteristic (ROC) would be established and do statistic analysis.Results:In hemolytic group(16cases), COHb were significantly greater than that of non-hemolytic group(16case)(p<0.05),but there weren’t statistic difference in HB, Ret and STB between two groups(p>0.05). Finally24cases got their ETCO results. In hemolytic group(10cases), COHb and ETCO were significantly greater and HB were lower than that of non-hemolytic group(14case)(p<0.05). However, there weren’t statistic difference in Ret and STB between two groups(p>0.05).The AUC were83.6%for COHb,77.9%for ETCO and75%for HB respectively(p<0.05). The cut-off value of COHb for optimum prediction of HDN was1.85%(sensitivity was80.0%) and that of ETCO was2.025ppm (sensitivity was90.0%).Their sensitivity were higher than that of HB.Conclusions:Endogenous Carbon Monoxide are more sensitive in diagnosis of hemolytic disease of the newborn than traditional hemolysis tests. COHb and ETCO can be new early screening methods.
