Abstract


The purpose of the preterm children patent ductus arteriosus (PDA) occurred on the relationship between risk factors and serum cortisol levels and premature PDA. Methods January 2008 to December were retrospectively analyzed 751 cases of premature children in Fujian Province Maternal and Child Health Hospital neonatal inpatient, 48 patients were diagnosed PDA case group, 85 cases of non-PDA for the control group, two groups of clinical information control study group Logistic regression model for the analysis of risk factors; Second, the reference to the 1:1 matching nested case-control study, on the basis of univariate analysis, application, select the period January 2009 to December born 432 preterm children in Fujian Province Maternal and Child Health Hospital neonatology treatment cohort, the pre-acquired the radial artery, was born 3 days after the diagnosis of PDA cases, in accordance with similar gestational age (gestational age difference of less than 1 week), the same mother prenatal dexamethasone to promote fetal maturity history matching, select the same period in hospital the same day echocardiography to exclude PDA in preterm children as the control group, a total of 38 pairs of matching. Nested case-control study: demographic characteristics and birth serum cortisol values ??influencing factors, no significant difference between the case and control groups (P are gt; 0.05) PDA group was born when serum cortisol concentrations the 9.49 ± 8.32μg/dl, while the control group was 13.76 ± 8.56μg/dl, the difference was statistically significant (P = 0.031). Logistic regression analysis showed that low birth serum cortisol levels is premature child PDA risk factors (OR = 0.916). Conclusion preterm infants with PDA with perinatal variety of factors, small gestational age, low birth weight, birth asphyxia, sepsis, low serum cortisol levels of birth is a major risk factor for mothers with prenatal hormones, oxygen is the main protective factors . The occurrence of preterm children PDA does not affect the serum cortisol levels. Targeted for perinatal care is to reduce the effective measures of PDA in preterm children.
