Objective:Diabetic Retinopathy (DR) is one of the most common complications in diabetic microvascular disease. With the increasing incidence of diabetes, DR becomes an important eye diseases which damages the eyesight of middle and old age people and serious influence the patient’s quality of life. In addition to sustained high blood sugar leads to a series of metabolic abnormalities, genetic factors is an important factor to the DR. From now,they has found a variety of gene polymorphism that is associated with the DR. Peroxisome proliferator-activated receptor (PPAR) is a kind of nuclear transcription factor, which is participate in the regulation of lipid and carbohydrate metabolism, and cell apoptosis in the body. Prol2Ala polymorphism of peroxisome proliferator activated receptor-y2(PPAR-y2) gene is considered play an important role in the type2diabetes mellitus (T2DM) and diabetic nephropathy (DN). It can increase the sensitivity of peripheral tissue insulin sensitivity, improve insulin resistance, regulate the metabolism of blood sugar and blood fat, promote adipocyte differentiation, as well as participate in anti-inflammatory and oxidation resistance reactions. Because the DR and the DN has many similarities in pathology and pathogenesis, so we infer that PPAR-γ2gene Prol2Ala polymorphism is associated with the occurrence of DR. Therefore, we through research the relationship between PPAR-y2gene Pro12Ala polymorphism and type2diabetic in shanxi of the han Chinese population. So that to explore the possible genetic factors of diabetic retinopathy and pathogenesis. And provide prevention and treatment basis of DR.Method:Totally90patients with T2DM were selected into our research, who were in the first hospital of shanxi medical university, at the age of40to70years old. diabetic duration from10to20years, blood pressure<140/90mmHg, without diabetic nephropathy, and without any blood relationship between individuals. According to eye exam results, it can be divided into non-DR (NDR) group, nonproliferative-DR (BDR) group and proliferative-DR (PDR) group, and each group has30cases. Collect peripheral blood in patients, and divided into2copies. One used to measure the fasting glucose, glycosylated hemoglobin, total cholesterol and triglycerides. And another extracts leucocyte DNA. The genotypes of Prol2Ala polymorphism of PPAR-y2gene were determined by polymerase chain reaction-restriction fragment length polymorphisms (PCR-RFLP) assay in all the patients. Experimental data using SPSS13.0statistics software to carry on the analysis, comparison of count data by chi-square test, the comparison between the measurement data used single factor analysis of variance, and Logistic multivariate regression is adopted to analysis the related factors, P<0.05is statistically significant.Results:The PCR showed that two alleles and three kinds of genotypes are exist in the study population. The frequency of genotype PP, PA, AA were40.0%,53.3%.6.7%in NDR group,70.0%,30.0%,0.0%in BDR group,76.7%,23.3%,0.0%in PDR group, respectively. P and A allele frequency in the NDR group were66.7%,33.3%, BDR group were85.0%,15.0%, in the PDR group were88.3%,11.7%. Comparison between the three groups, genotype frequency (x2=10.351) and allele frequency (x2=10.208) difference was statistically significant (P<0.05). Logistic regression analysis results showed that the Prol2Ala polymorphism(p=0.007, OR=0.261,95%CI0.098-0.694), glycosylated hemoglobin(P=0.013, OR=1.459,95%CI1.082-1.967) and serum total cholesterol(P=0.013, OR=1.895,95%CI1.147-3.130) are independent risk factors for DR occurred.Conclusion:(1) There is Prol2Ala polymorphism of PPAR-γ2gene in Chinese Han people in Shanxi Province;(2) The alanine variant of Prol2Ala polymorphism of PPAR-y2gene is associated with DR in type2diabetes among the Hans in Shanxi
