Abstract


Objective:clinical observation on quality on the recovery of pediatric laparoscopic operation after operation monitoring by Narcotrend and BIS and the feasibility of monitoring by Narcotrend and BIS in pediatric, for getting the more clinical data on the research of children for monitoring depth of anesthesiaMethods:select60cases of pediatric patients undergoing laparoscopic operation, including:the high ligation of hernia sac, hydrocele of tunica vaginalis ligation, appendectomy, cryptorchidism testis down operation, high ligation of spermatic vein,6-12years old,they were randomly divided into A group and B group, each group had30cases. Monitoring instrument:A group used Narcotrend (group N); group B used BIS (group B), Anesthesia methods used intravenous-inhalation anesthesia Maintenance of anesthesia aplly to sevoflurane and remifentanil in the operation, all patients in the elimination of pneumoperitoneum ceased sevoflurane and in Sewing leather last stitch stoped the remifentanil infusion, In the end of operation we carried out the determination of monitoring data through postoperative sputum suction of score trachea (tracheal suctioning score, TSS), Ramsay score and recovery of consciousness (that is no reaction to a stimulus, Calling can make patients reacted and eyes, orientation recovery) corresponding to each case in two groups.Results:There had no significant statistical difference in gender, age and body weight through the study of two groups in children (P>0.05);the studies showed that NT values and and BIS values had increased with the decrease of TSS score,That NT group picks up in speed and amplitude was significantly higher than that in BIS group, NT value and BIS value had significant statistical difference during TSS1-5score in the two group (P<0.05); similarly, NT value and BIS value also increased with that the corresponding Ramsay sedation score had decreased from1to6in the two groups, NT group changed more significant in the speed and the amplitude than that in BIS group in the Ramsay sedation score partition from5to6and in the Ramsay sedation score partition from1to3,and the difference of NT value and BIS value had statistical significance corresponding to two samples of the same Ramsay score point (P<0.05); the NT value and BIS value that corresponding to the Ramsay4had no significant difference (P>0.05); in addition, the NT value and BIS value corresponding to the recovery of awareness of children had increased, NT value and BIS value from L1or from L2had no statistical significance differences (P>0.05); and the velocity and amplitude of the child recovered directional force in NT group was significantly higher than that in BIS group, the NT values corresponding to L3observation point had more statistical difference than BIS value (P<0.05).Conclusion:There were a significant correlation in postoperative recovery quality between NT value and BIS value, they could effectively reflect the change of the level of sedation and the level of consciousness recovery in the6-12years old children in laparoscopic operation during anesthesia recovery. NT value and BIS value of the recovery period after surgery in children had changed with TSS score, Ramsay sedation score and the degree of consciousness recovery. Compared with BIS,that NT value declined in the deep anesthesia state slowed down, and NT value had rised faster in the period of recovery than BIS value,it had reflected that NT was better than BIS in the performance reflecting quality of recovery.
