The first part of the canine achalasia model Objective: To establish a canine model of achalasia, for the new temporary cardia metal stents achalasia experimental study conditions. Methods: 24 adult beagle dogs, under endoscopic guidance tetradecyl - dimethyl - benzyl ammonium chloride (benzyl-dimethyltetradecylammonium chloride, BAC, 4 mmol / L, 12 mL) was injected into the lower esophageal circumferential sphincter (lower esophageal sphincter, LES). BAC was observed after the clinical manifestations of experimental animals. BAC drug treatment before and after 6 mon respectively esophageal manometry and timing of esophageal barium (timed barium esophagogram, TBE), to evaluate the success of modeling. Results: BAC before and after treatment in dogs LES resting pressure has a very significant statistical difference (22.88 ± 3.13 mmHg vs 47.14 ± 6.12 mmHg; P lt; 0.001). TBE dogs before processing the first 5 min of barium display width, height, and the product was 0.96 ± 0.55 cm, 3.87 ± 2.19 cm and 4.55 ± 3.74 cm2, respectively, after treatment 3.04 ± 0.47 cm, 9.74 ± 1.33 cm and 29.39 ± 4.35 cm2, between the two groups there was a significant statistical difference (both P lt; 0.001). After eating to reduce the incidence of 62.5% (15/24), returning food (vomiting) occurred in 50% (12/24), weight loss rate was 54.2% (13/24). Experimental animals did not occur during the experiment deaths. Conclusion: Endoscopic BAC LES circumferential injection drugs can create an ideal achalasia canine model. The second part of the new temporary metal stent treatment of canine cardiac achalasia results Objective: To explore new temporary metal stent treatment of canine cardiac achalasia efficacy and complications, follow-up study stent after removing different time points esophageal histopathological changes characteristic segment. MATERIALS AND METHODS: cardiac injection tetradecyl - dimethyl - benzyl ammonium chloride (benzyl-dimethyltetradecylammonium chloride, BAC, 4 mmol / L, 12 mL), on 24 experimental Beagle dogs achalasia disease modeling work, a total of 24 successful modeling and randomly divided into control group (n = 12), the experimental group (n = 12), respectively, normal esophageal stent placement and new temporary cardiac stent; indwelling stent removed after four days . Mouse anti - PCNA monoclonal antibody immunohistochemical staining showed the epithelium were hyperplastic inflammation peaked within 2-4 wk, no statistically significant difference between the groups. Mouse anti-α-SMA monoclonal antibody immunohistochemical staining showed submucosal smooth muscle proliferation were reached peak within 2-4 wk, no statistically significant difference between the groups. Masson trichrome staining showed submucosal collagen fibers proliferative activity gradually increased and stabilized at 3 mon state when no statistical difference between the groups. Conclusion: In this study, a new temporary treatment of canine cardiac stent is safe and effective for achalasia, esophageal stents more effective than ordinary, but the sample size needs to be larger and longer follow-up time of basic experimental and clinical research.
