Abstract

Objective: To understand the region clinical isolates of Pseudomonas aeruginosa MexAB-OprM type efflux system and membrane pore protein OprD2 in the expression of its Multiple resistance with Pseudomonas aeruginosa clinical anti-infection treatment and hospital infections monitoring and control of evidence-based medicine. My the hospitalized patients submission specimens separation methods: July 2008 to June 2009 Pseudomonas aeruginosa 381 (similar specimens of the same patient isolates are not counted). VITEK 2 Compact automatic microbial analyzer for bacterial identification and susceptibility determination. Select multi-drug resistant Pseudomonas aeruginosa (three categories or more antibiotic resistance), respectively broth microdilution test for detection of MexAB-OprM active efflux system, the quantitative PCR technique efflux system and OprD2 relative quantitative determination the the high expression MexAB strain regulating gene mexR for PCR amplification, and the products were sequenced. Results: 1. Topped Pseudomonas aeruginosa infection in children and the elderly in the various sections of the hospital, the ICU patients with Pseudomonas aeruginosa separation rate (14.2%) from the site of infection, mainly distributed in the respiratory tract (62.2%). Pseudomonas aeruginosa resistance situation is very serious, and have a higher resistance to the most commonly used antibiotics, piperacillin / tazobactam, amikacin, kanamycin and other antibiotics are still the treatment of Pseudomonas effective antimicrobial false Aeromonas infection. Proton pump inhibitors CCCP antibacterial effect of ciprofloxacin greater impact, 15 Pseudomonas aeruginosa MIC dropped more than eight times, including six CAZ in CCCP exist MIC also fell eight times above, and another two, only the MIC of ceftazidime dropped more than 8 times. Imipenem antimicrobial activity not subject to the influence of the CCCP. Ceftazidime resistant and sensitive of the two groups of Pseudomonas aeruginosa MexAB-OprM active efflux system expression levels were significantly different (P lt; 0.05), ciprofloxacin resistant and sensitive of the two groups of Pseudomonas Pseudomonas MexAB-OprM active efflux system expression levels were significantly different (P lt; 0.05), but resistance to imipenem and sensitive of the two groups, the amount of this type of expression of the efflux system is no difference ( P gt; 0.05). 5. MRNA relative quantification with ceftazidime of CCCP inhibition better consistency of the test results, ciprofloxacin inhibition test positive strains, 7 relative quantification. 6. The membrane porin OprD2 gene mRNA quantitative test results show that the expression of the imipenem group below the sensitive group, and the difference was statistically significant (P lt; 0.05) Conclusion: 1. Pseudomonas aeruginosa Aeromonas the hospital infection pathogens display multi-drug resistant, it should be a reasonable choice antibiotics. 2 high expression of active efflux system of Pseudomonas aeruginosa MexAB-OprM type multidrug-resistant mechanism. MexAB-OprM active efflux system high expression and Pseudomonas aeruginosa CIP and CAZ resistance, rather than the drug mechanism of IMP. 4. OprD2 decreased expression is an important mechanism of IMP appear resistant Pseudomonas aeruginosa.
