Abstract


PartⅠUS-guided diffused optical tomography:an initial clinical application in breast lesionsPurposeTo investigate the feasibility of using US-guided diffused optical tomography(DOT) in differentiating breast malignant lesions from benign lesions and evaluate its clinical efficacy.Materials and methodsFrom October 2007 to August 2008,patients scheduled for open biopsy at Peking Union Medical College Hospital were included.First,conventional US was performed at each patient,all the US features(including Color Doppler Flow Imaging) were recorded.Then grayscale US was used to guiding location of the breast lesion,diffused optical tomography was performed.Of each visible breast lesion detecting by US,total hemoglobin concentration(THC) was acquired from DOT.Sensitivity,specificity and overall accuracy were determined with surgical pathology as the gold standard.ResultsThere were 96 benign lesions and 118 malignant lesions in 214 breast lesions.THC of all lesions was acquired.The average THC of benign lesions was 125.5μmol/L±83.4,and 222.2μmol/L±87.2 of malignant lesions respectively.The THC of malignant lesions was significantly higher than that of benign lesions(p＜00.05).There was no significant difference of average THC in benign groups,such as fibroadenoma group,adenosis group, and intraductal papilloma group(p＞00.05).There was no significant difference of average THC in malignant groups,such as invasive ductal carcinoma group,ductal carcinoma in situ group,and invasive lobular carcinoma group(p＞00.05).When the THC of 140μmol/L was used as the cutoff value for differentiating breast cancer from benign lesions,the sensitivity,specificity,accuracy,positive predictive value,negative predictive value of DOT were 83.9%,66.7%,76.2%75.6%,77.1%respectively.The sensitivity,specificity,accuracy, positive predictive value,negative predictive value of conventional US were92.4%,71.8%, 80.1%,88.5%,83.2%.Moreover,combined conventional US with DOT using for identifying breast cancer from benign lesions,the specificity could be increased to 85.4%, and overall accuracy could be increased to 86.4%。ConclusionThere were significant difference of THC between benign and malignant groups,and the THC of malignant lesions was significantly higher than benign lesions.However,there were no significant difference of THC in inter-groups of various benign lesions,and no difference of THC in inter-groups of various malignant lesions.US-guided diffused optical tomography as a noninvasive functional imaging could be used to differentiate breast cancer from benign lesions.Combining conventional US with DOT could enhance the specificity and accuracy of diagnosis.
