Abstract

Because of its advantages,the peripheral intravenous cannulae are now widely used instead of traditional steel needle in many developed countries and also used in China for over 20 years.But a problem associated with the peripheral intravenous cannulation still exists.In order to improve the infusion quality,studies have been conducted to discover and solve these problems by nursing practioner from home and abroad.Objective:To describe and analyze the actual routines in the usage of peripheral intravenous cannula and the occurrence of complications in elderly patients in hospital.Make a preliminary evaluation of the improved nursing intervention for the elderly patients with peripheral intravenous cannula.Method:1.the observational study method was utilized to study the current status of peripheral cannula among elderly patients in the emergency intensive care unit of a tertiary general hospital of shanghai.The patients were followed from the time the cannula was inserted until withdrawal. Parameters included patient demographics,diagnoses,the site of inserted, dressing routine,dwell time,the occurrence of complications,ect.2. Own control was taken to evaluate the improved nursing intervention directed to the character of the elderly patients with peripheral intravenous cannulae. The study compared the outcome which includes dwell time,the occurrence of complications and the elderly patients’ acceptance of the peripheral intravenous cannulae before and after the improved and procedural infusion nursing intervention.Results:1.Thirty elderly patients were observed daily and seventy-one cannulations were recorded.Infusion nursing care were taken according to the peripheral intravenous cannulae caring procedure.The average dwell time was 3.59±1.24 days.Most of the cannulae were placed on the dorsal of the hand and forearms.The incidence of complication is 67.61%. Swelling(40.85%) and phlebitis(21.13%) were the most common complications. The type of peripheral cannula would reflect the appearance of the phlebitis, thinner cannula would reduce the incidence of phlebitis(p＜0.05).Patients who received emulsion were more likely have swelling(p＜0.05).2.Thirty-one elderly patients participated in this stage of the study and sixty-two cannulations were recorded before and after the improved and procedural infusion nursing inventions was taken.The average dwell time was prolonged while the period of cannulation is 4 days(3.34±0.79 days before and 3.85±0.44 days after intervention,p＜0.05).It also reduced the incidence of complications(62.90%before and 35.48%after intervention,p＜0.05).The incidence of swelling and phlebitis reduced(p＜0.05),and pain of venopuncture relieved(p＜0.05),but the intervention did not influence the rate of cannula occlusion and dislodgement(p＞0.05).In addition,pain of venopuncture was also relived(p＜0.05)Conclusion:1.The outcome of observation showed the incidence rate of complication is quite high.The elementary cares for the elderly patient with a peripheral cannulae were taken.2.Improved and procedural infusion nursing intvention helped to prolong the dwell time,reduce the occurance of complications and increas the acceptance of the peripheral intervenous cannulae in elderly patient.
